[Objective]Through retrospective analysis of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with type 2 diabetes(T2DM)in patients with clinical data,to investigate the acute exacerbation of chronic obstructive pulmonary disease with the clinical characteristics of the patients with type 2 diabetes,it provides some objective basis for clinical evaluation of AECOPD combined with T2DM.[Methods]1.The clinical data of AECOPD combined with T2DM in the Department of respiratory medicine of the First Affiliated Hospital of Kunming Medical University from January 2016 to January 2018 and the clinical data of simple AECOPD patients hospitalized in the same period were analyzed retrospectively. All AECOPD patients were in accordance with the diagnostic criteria of the GOLD guidelines(Revised Edition 2015);all T2DM patients were previously diagnosed with T2DM or random venous plasma glucose test>11.1 mmol/L or glucose test of fasting venous plasma>7.0mmol/L.2.The following items were compared between AECOPD combined with T2DM patients and simple AECOPD patients:age, sex, smoking history, hospitalization days, hospitalization expenses, weight, height, blood gas analysis,CRP,PCT,leukocyte, neutrophils percentage,hemoglobin,coagulation/fibrinolysis(PT,APTT,Fib),D-two polymer;Pulmonary function(FEV1%Pre)3.the above data were analyzed by SSPS21.0 system.Quantitative data were analyzed by mean + standard deviation(?ąS).Compared with X2 test between groups, two independent samples were tested by t test, and P<0.05 was statistically significant.[results]In this study, there was no significant difference in sex composition, smoking history;Hb,PCT,APTT,Fib between 60 AECOPD combined with T2DM patients and 60 cases of simple AECOPD(P>0.05).The two groups of age, hospital days, hospitalization costs, the number of antibiotics used, the number of use of antibiotics, the number of days of intravenous hormone use,the percentage of the estimated value(FEV1Pre%),PO2,PCO2,VBC,N%,CRP,D-D,PT,BMI were all statistically significant(P<0.05).[Conclusion]1.Hypoxemia and CO2 retention were more obvious in group AECOPD combined with T2DM than in group AECOPD.2.The levels of PT and D-D dimer in AECOPD group with T2DM were significantly higher than those in AECOPD group.3.The white blood cell count, neutrophil percentage and CRP level in group AECOPD combined with T2DM were higher than those in group AECOPD alone.4.The AECOPD group with T2DM needs more antibiotics, longer intravenous use of hormone, longer hospitalization time and higher cost of hospitalization than the simple AECOPD group.5.The level of FEVl%Pre in group AECOPD combined with T2DM was significantly lower than that in patients with simple AECOPD.
