Objective: To explore the correlation between serum free carnitine content and the severity and prognosis of acute myocardial injury.Methods: We collected 51 cases of acute myocardial injury patients(acute myocardial injury group)in Medical Intensive Care Unit and 52 cases of non-acute myocardial injury patients(control group)in the internal medicine general wards of Zhongshan Hospital affiliated with Dalian University.We collected their general information,including name,sex,age,underlying disease,hypersensitivity myoglobin I,electrocardiogram,28-day prognosis of hospitalization,24-hour acute physiology and chronic health score(APACHE II score),and tested their serum fatty acid metabolism spectrum(free carnitine)24 hours after hospitalization.Comparing serum free carnitine content between the acute myocardial injury group and the control group,we found that they had statistically significant differences.The acute myocardial injury group was divided into Group A(0<APACHE II?15),Group B(15<APACHE II?30)and Group C(APACHE II>30).The differences and correlations between serum free carnitine content and hypersensitive troponin I(hs-c Tn I)content were analyzed.According to 28-day survival prognosis,we divided the acute myocardial injury group into survival group and death group,and analyzed whether they are correlated.Results: 1.The content of serum free carnitine in acute myocardial injury group(n=51)was(24.93±9.90)?mol/L,and that of serum free carnitine in control group(n=52)was(31.22±10.77)?mol/L.They had statistically significant differences(p<0.05).2.There is a negative correlation between serum free carnitine content(24.93 ± 9.90)?mol/L and serum hs-c Tn I content(0.57±0.18)ng/m L in acute myocardial injury group(r=-0.287,p=0.041<0.05).The former has a negative correlation with APACHE II score(25.88 ±9.81)(r=-0.420,p=0.002<0.05).The result had statistically significant differences.3.According to APACHE II score,the acute myocardial injury group was divided into Group A(n=10),Group B(n=21)and Group C(n=20).The content of serum free carnitine in the three groups were(30.11±12.19)?mol/L in Group A,(27.17±9.59)?mol/L in Group B,and(19.99±6.74)?mol/L in Group C.The three groups of serum free carnitine content had statistical differences(H=9.451,p=0.009<0.05).The content of serum hs-c Tn I in the three groups were(0.44±0.13)ng/m L in Group A,(0.57±0.14)ng/m L in Group B,and(0.67±0.19)ng/m L in Group C.The three groups of serum hsc Tn I had statistical differences(H=13.306,p=0.001<0.05).4.According to 28-day survival prognosis,the acute myocardial injury group was divided into survival group(n=33)and death group(n=18).The content of serum free carnitine were(26.67±10.56)?mol/L in survival group,and(21.75±7.86)?mol/L in the death group.The result showed no evident differences between the two groups(t=1.729,p=0.090>0.05).The APACHE II score of survival group was(21.88±7.97)and that of death group was(33.22±8.67).The results of the two groups had significant difference in the APACHE II score(t=-4.710,p=0.000<0.05).The content of serum hs-c Tn I in survival group was(0.53±0.15)ng/m L and that of death group was(0.64±0.21)ng/m L.The result showed no differences between the two groups(t=-2.165,p=0.035<0.05).Conclusions: The content of serum free carnitine in acute myocardial injury patients were lower than the patients in non-acute myocardial injury.The lower the level of serum free carnitine in patients with acute myocardial injury,the more severe the myocardial injury.Therefore,the content of serum free carnitine may be a biomarker to evaluate the severity of the condition in patients with acute myocardial injury.
