Objective Analyze the status quo of self-management behavior and the influencing factors of self-management ability of stroke hemiplegic patients through cross-sectional investigation;use behavior transformation theory model and self-management model theory to discuss the rehabilitation training of cognitive behavior intervention in stroke hemiplegic patients' selfmanagement behavior effect.Methods The first phase of the baseline survey: 224 patients with stroke hemiplegia who were hospitalized in Jiangbin Hospital of Guangxi Zhuang Autonomous Region from December 2018 to March 2019 were selected as the research object.The survey was conducted from four aspects: general demographic data,social support,stroke knowledge,and self-management,using single factor analysis,Pearson correlation analysis,and multiple linear regression to analyze the influencing factors of self-management behavior of stroke hemiplegic patients.To guide the development of personalized selfmanagement behavior intervention programs.The second stage: 84 patients with stroke and hemiplegia who met the inclusion criteria and were hospitalized in Jiangbin Hospital of Guangxi Zhuang Autonomous Region from April to December 2019 were selected as the research object.They were divided into intervention group and control group according to random number table method.In 42 cases,the control group adopted routine nursing methods;the intervention group used behavioral change theory and self-management theory models on a routine basis to construct a personalized rehabilitation training contract and common and personal cognitive behavioral interventions for stroke-based hemiplegic patients Measures.The intervention training time was 4 weeks,5 times/week,20?30 min/time.Observe the five indicators of self-management ability,daily living ability(Barthel index),physical motor function activity ability,motor self-efficacy,and functional exercise compliance of the two groups of patients after 1 month,3 months,and 6 months of intervention The debate also observed the incidence of complications after 6 months of intervention(swollen limbs,shoulder pain,pressure ulcers,foot prolapse,shoulder dislocation),and explored the changes in self-management behaviors of cognitive behavioral interventions in stroke hemiplegic patients Rehabilitation training effectiveness,verify the clinical application of problem-oriented personalized self-management behavior interventions.Results Univariate analysis of patients' BMI index,education level,whether they work,family monthly income per capita,place of residence,smoking history have an impact on self-management behavior(P <0.05),Pearson correlation analysis of each dimension of self-management behavior and total score and stroke Knowledge,attitude,and social support are all positively correlated(correlation coefficients r = 0.513,0.519,0.708).Multiple linear regression analysis showed that the differences in education level,social support,stroke attitude,monthly income per capita,residence,and stroke knowledge were the influencing factors of hemiplegic patients' self-management behavior(P <0.05).The differences were statistically significant.After 1 month,the intervention group's daily living ability,exercise selfefficacy,and functional exercise compliance were all better than the control group(P< 0.05);the self-management behavior and daily life after 3 and 6 months The five indicators of ability,limb motor function,exercise selfefficacy,and functional exercise compliance were superior to the control group(P <0.05),while the incidence of complications such as swelling of the limbs,shoulder pain,and prolapse of foot drop was lower than the control group(P <0.05),the difference was statistically significant.The results of repeated measurement analysis of variance show that the spherical symmetry test result P<0.05,indicating that there is a certain correlation between the repeated measurement results without passing the spherical test.The Greenhouse-Geisler correction results are used,and the group shows F=3.256,P <0.045,indicating that the self-management capabilities of different groups are not the same;the time results show F=703.295,P<0.001,indicating that the difference between each time point is statistically significant;the interaction effect of time*group shows F=40.077,P<0.001.It shows that there is an interactive effect between time and method,different groupings,different times,different intervention effects of selfmanagement ability.The change trends at the four time points show that the intervention group and the control group both increased with time,and the five rehabilitation indicators such as self-management behavior,daily living ability,limb motor function,exercise self-efficacy,and functional exercise compliance also gradually increased,And the patients in the intervention group recovered better in the same time.Conclusion(s)Indicators such as the patient's education level,social support,stroke attitude,family monthly income per capita,place of residence,and stroke knowledge are factors that influence the self-management behavior of stroke hemiplegic patients.This study uses behavioral change theory model and self-management model theory to build a personalized rehabilitation training contract and cognitive behavior intervention measures for stroke hemiplegic patients with problem-oriented,which can improve patients' selfmanagement behavior,daily life ability,limb motor function Exercise selfefficacy and functional exercise compliance reduce the incidence of stroke hemiplegia complications and improve the patient's quality of life.
