Research On Constructing The Mode Of Hospice Care For Cancer Patients Based On MDT

[Objective]1.To develop a questionnaire for hospice care needs of cancer patients in accordance with Chinese culture,and describe their hospice care needs.To provide tools for comprehensive,systematic and long-term dynamic assessment of cancer patients.2.Through qualitative research to understand and analyze the care burden and demand of caregivers of hospice care patients with end-stage cancer,the problems and development direction of hospice care for cancer patients in China were discussed.3.To construct the care model for promoting hospice care for cancer patients,to provide a scientific and effective program for the clinical practice of hospice care for cancer patients in China,to promote the development process of hospice care in China,and to provide theoretical thinking for exploring the construction of doctor-patient community with a Shared future.[Methods]1.Based on the Omaha system theory,developed by two rounds of Delphi enquiry for cancer patients,hospice care needs a questionnaire by the method of convenient sampling,198 cancer patients clinical investigation,using the SPSS 22.0 software to obtain the data statistical analysis,through the evaluation of the internal consistency of the questionnaire,retest reliability,structure validity and content to verify the reliability and validity of the questionnaire is scientific and practical.2.By means of semi-structured in-depth interviews in qualitative research and intentional sampling,10 caregivers of end-stage cancer patients in hospice care ward of a hospital in***were selected as research objects,and the 7-step analysis method proposed by Colaizzi was used to extract meaningful narratives and conduct coding classification,interpretation and comprehensive analysis.3.On the basis of theoretical research,investigation research and qualitative research,the care model and operation framework for promoting hospice care for cancer patients were constructed.[Results]1.Results of Delphi questionnaire on hospice care needs of cancer patients:in the end,a total of 16 experts completed two rounds of questionnaire,the expert's authority coefficient was 0.83,the recovery rates of the two rounds of questionnaire were 88.89%and 100%,and the Kendall coefficient was 0.306 and 0.362,respectively.A total of 3 items were deleted,3 items were added and 7 items were modified.A questionnaire on hospice care needs of cancer patients with 36 items in 4 dimensions was initially formed.2.Results of the survey on hospice care needs of cancer patients:220 questionnaires were issued in this survey,198 questionnaires were effectively recovered,and 6 factors were generated by exploratory factor analysis,with the contribution rate of cumulative variance being 70.563%.Questionnaire content validity is 0.933,each objective content validity is 0.813?1.000,the questionnaire Cronbach 's coefficient is 0.934,retest reliability is 0.907,eventually form contains 28 items of questionnaire,the study found that patients with cancer at the same time the overall demand for hospice care to belong to medium and high level,the need to be accounted for the highest degree of physiological needs,including sleep and pain management was particularly strong demand.3.Qualitative research results on care burden and needs of caregivers of hospi patients:a total of 4 subjects were summarized,including affected living conditions,obvious negative psychological experience,desire for professional hospi multidisciplinary teams,and desire for care-related knowledge and skills.9 sub-themes:physical decline,limited social activities,uncertainty,lack of emotional support,worry about the future,hope for regular psychological counseling,hope to provide spiritual care,hope to improve the caregiver's care skills,and desire for guidance in home palliative care.4.The results of constructing a hospice care model for cancer patients based on MDT:formed taking patients as the center,the family as the main body,requirements and acceptance as the guidance,meet the multidimensional comfortable nursing as the goal,the multidisciplinary collaboration to promote cancer patients hospice care patterns of operation framework,build the mode of the concrete content includes the policy basis,main objectives,organizational structure,inclusion criteria,access process,basic elements,etc.[Conclusions]In this study,a questionnaire on hospice care needs of cancer patients was prepared through Delphi expert's letter consultation.The reliability and validity of the questionnaire was tested through a large sample survey,and the degree of hospice care needs of cancer patients was discussed,which provided an assessment tool for the development of personalized hospice care services.Qualitative interviews with caregivers of hospice care patients with end-stage cancer were used to deeply analyze the caregivers' nursing experience,explore the development direction of hospice care for cancer patients,and provide a reference for the construction of a model of hospice care for cancer patients.The model of promoting palliative care for cancer patients is scientific and reasonable,which has important guiding significance for improving the quality of life of patients with end-stage cancer and their families.In the future,the model of palliative care for cancer patients should be further verified and improved to improve the quality of palliative care for cancer patients in China.
