Objective: To study the effect of acupotomy on general condition,body weight,body length,waist circumference,Lee's index,NPY / POMC and leptin in hypothalamus of obese rats.It is proved that Acupotomology can not only control the body weight of simple obesity rats,but also stimulate the hypothalamic energy regulation center and improve the internal environment of simple obesity patients,which proves the advantages of minimally invasive Acupotomology in the treatment of simple obesity,in order to provide experimental basis for the clinical application of acupotomology.Methods: 70 male SD rats were selected and fed for 1 week.10 rats were selected as the blank group by random number method and fed with common feed.The rest rats were fed with high-fat feed to prepare the simple obesity rat model.All rats were fed and drank freely.The feed and water were changed once a day.The body weight,body length and waist circumference were measured once a week.The Lee's index was calculated.The Lee's index and the rate of weight gain could also be used as an important index to reflect the degree of obesity.After being fed with high fat diet for 16 W,the body weight of the rats was more than 20% of the average body weight of the normal group,which was called the obese rats group(DIO).Randomly divided into model group,Acupotomology group and Western medicine group.Blank group(n = 10): normal feeding,no treatment,a total of 4 weeks.Model group(n = 10): high fat diet,no treatment,a total of 4 weeks.Acupotomotor group(n = 10): the first treatment(week 1)was located in the rat's t6-10 spinous process,and the tough part was dredged longitudinally and stripped horizontally by 2-3 knives;the second treatment(week 2)was located in the rat's T6-T10 left articular process capsule,with a 0.1cm paravertebral opening,The third treatment(the third week)was located at the right articular capsule of T6-T10 in rats,with a 0.4mm × 40 mm needle knife to enter the needle vertically,to reach the tenacity,then to conduct longitudinal dredging,and to conduct transverse stripping for 2-3.In the fourth treatment(week 4),"Zhongwan" point,"Tianshu" point in the middle abdomen and "Guanyuan" point in the lower abdomen were injected vertically with a 0.4mm × 40 mm needle knife,and then the fat layer was removed twice.Compression hemostasis to prevent infection after treatment.Western medicine group(n = 10): the clinical dosage of orlistat was 360 mg / D,and the equivalent dosage of rats was 360 * 0.018 * 5 = 32.4mg/kg/d.The drug concentration was calculated to be 3.24mg/ml according to the dosage of 4m1 per day.One Jasmine orlistat capsule(120mg)can be dissolved in 37 ml pure water to obtain gastric perfusion solution.Once a day for 4 weeks.Four weeks later,four groups measured body length,body weight and waist circumference.Rats in each group were perfused with paraformaldehyde for internal fixation.Hypothalamus,liver,fat and other tissues were put into paraformaldehyde solution for standby.He staining was used to observe the morphological changes of liver and adipose tissue,and immunohistochemistry was used to detect the expression of NPY / POMC and leptin in hypothalamus.Results:(1)the body weight of rats in the acupotomotor group decreased significantly after treatment compared with the success of modeling(P < 0.05).(2)The body weight,waist circumference,testis circumference,kidney circumference and brown fat content of the acupotomotor group were significantly improved compared with the model group(P < 0.05),but the body length was not significantly changed.(3)He staining showed that after acupotomy,the number of lipid droplets in hepatocytes decreased significantly,the arrangement of liver tissue was more compact,the number of fat cells decreased significantly,and the arrangement was more orderly.(4)After acupotomy,the expression of NPY decreased significantly(P < 0.05),while that of ?-MSH and LP decreased significantly(P < 0.05).Conclusion: The therapeutic effect of acupotomy on simple obesity rats is obvious,which is closely related to the expression level of NPY,POMC,LP and other factors in the hypothalamic energy regulation center,and has less side effects than western medicine.
