Clinical Research Of Needle Warming Moxibustion With Different Number Of Cones In Treating Cold-dampness Syndrome Knee Osteoarthritis

ObjectiveTo compare the changes of WOMAC scale score,Lysholm score and quality of life SF-12 score before and after treatment with needle warming moxibustion with two cones and needle warming moxibustion with one cone in two groups of patients with knee osteoarthritis(KOA)of Cold-dampness Syndrome by clinical randomized controlled study,so as to compare the clinical efficacy of needle warming moxibustion with two cones and needle warming moxibustion with one cone on KOA of Cold-dampness Syndrome,and provide basis for the selection of needle warming moxibustion cone in treatment of KOA of Cold-dampness Syndrome.Methods64 KOA patients who met the inclusion criteria and exclusion criteria visited Acupuncture Department of Guanggu Branch of Hubei Provincial Hospital of TCM from September 2018 to September 2019 were selected as the study subjects,and the 64 patients were divided into Group of needle warming moxibustion with two cones and needle warming moxibustion with one cone by random number table method randomly.Patients of Group of needle warming moxibustion with one cone lied in supine position,with their Xuehai,Liangqiu,Dubi,Neixiyan,Yanglingquan and Zusanli acupunctured and the moxa cone inserted into the needle handle after acuesthesia.Group of needle warming moxibustion with two cones continued to be treated after needle warming moxibustion with one cone,and was treated with a total of needle warming moxibustion with two cones.Group of needle warming moxibustion with two cones and needle warming moxibustion with one cone were treated once every other day,3 times a week,with every 2 weeks as a course of treatment,treated for 2 courses of treatment continuously,and were followed up 8 weeks and 12 weeks after admission respectively.Patients were required to fill in WOMAC Scale Table,Lysholm Scale Table and Quality of Life SF-12 Scale Table before treatment,4 weeks after treatment,8 weeks after admission and 12 weeks after admission respectively,and scores of patients before and after treatment were recorded.The statistical data were analyzed by SPSS23.0 software to evaluate the clinical efficacy of the two groups in treating KOA of cold-dampness syndrome,including evaluation of short-term and long-term efficacy.In addition,the safety of warming acupuncture in treatment of the disease was evaluated.ResultsA total of 62 patients finished treatment and acupunctures of 2 patients fell off.1.Comparison of baseline data between the two groups of patients: the age,gender,course of disease,disease location and other aspects of the two groups of patients were compared.There was no significant difference(p > 0.05),the two groups were comparable.2.Comparison within each group of patients 4 weeks after treatment: the 2 groups of patients within the baseline period were compared.After 4 weeks of treatment,patients' WOMAC pain score,stiffness score,difficulty score in daily activities and the total score decreased significantly,Lysholm score and SF-12 score increased significantly,and the difference was statistically significant(p < 0.01).3.Comparison between the two groups of patients 4 weeks after treatment: group of needle warming moxibustion with two cones is superior to group of needle warming moxibustion with one cone in reducing patients' WOMAC pain score,stiffness score,difficulty score in daily activities and the total score and improving Lysholm score and SF-12 score,and the difference was statistically significant(p < 0.05).4.Clinical efficacy rate: after 4 weeks of treatment,in Group of needle warming moxibustion with two cones,6 cases were cured,21 cases were improved,2 cases had no efficacy,and its total efficacy rate in the treatment of KOA of cold-dampness syndrome was 93.5%;in group of needle warming moxibustion with one cone,4 cases were cured,19 cases were improved,8 cases had no efficacy,and its total efficacy rate in the treatment of KOA of cold-dampness syndrome was 74%.The efficacy rate of group of needle warming moxibustion with two cones was significantly higher than that of group of needle warming moxibustion with one cone,and the difference between the two groups was statistically significant(p < 0.05).5.Long-term efficacy: the two groups of patients were compared in the 8th week after admission and the 12 th week after admission.The 2 groups had significant efficacy in reducing patients' WOMAC pain score,stiffness score,difficulty score in daily activities and the total score and improving Lysholm score and SF-12 score.The efficacy of group of needle warming moxibustion with two cones was significantly superior to that of group of needle warming moxibustion with one cone in reducing the WOMAC pain score,stiffness score and improving Lysholm score and SF-12 score(p < 0.05).In the 8th week after admission,there was no significant difference between the 2 groups in reducing WOMAC patients ' difficulty score in daily activities(p > 0.05).In the 12 th week after admission,there was no significant difference between the 2 groups in reducing WOMAC patients' difficulty score in daily activities and the total score(p > 0.05).6.Safety evaluation: During the whole process of this study,there was no occurrence of any adverse events such as scald,fainting during acupuncture,fainting during moxibustion,sticking of needle,needle breakage,etc.in the 2 groups.Conclusion 1.In treatment of KOA of cold-dampness syndrome in group of needle warming moxibustion with two cones and group of needle warming moxibustion with one cone,from the perspective of short-term efficacy,they had good efficacy in relieving patients ' pain,stiffness and difficulty in daily activities,improving knee joint function and improving patients' quality of life,and group of needle warming moxibustion with two cones was superior to group of needle warming moxibustion with one cone in efficacy.2.From the perspective of long-term efficacy,they had good efficacy in relieving patients' pain,stiffness,improving knee joint function and improving patients' quality of life,and Group of needle warming moxibustion with two cones was superior to Group of needle warming moxibustion with one cone in efficacy,but the long-term efficacy of both groups were equivalent in reducing difficulty in daily activities.In general,the efficacy of Group of needle warming moxibustion with two cones was superior to that of group of needle warming moxibustion with one cone,which could provide a reference basis for the selection of needle warming moxibustion cone in treatment of KOA of cold-dampness syndrome.
