| Objectives: In this study,we compared the clinical efficacy of electroacupuncture of Jiaji combined with blood-letting puncture and cupping and oral administration of drugs in the treatment of Postherpetic Neuralgia(PHN).Through the detection and analysis of the changes in the patient's serum Interleukin 6(IL-6)and necrosis factor?(TNF-?)level before and after treatment of 2 groups,to clarify the advantages of electro-acupuncture of Jiaji combined with bloodletting puncture and cupping in the treatment of postherpetic neuralgia and its cellular immune regulation,It provides a scientific and reliable theoretical basis for the clinical application of electroacupuncture of Jiaji combined with bloodletting puncture and cupping for postherpetic neuralgia.Methods: All the cases in this study were from October 2017 to January 2019 in Wuhan Integrated Chinese and Western Medicine Hospital and were derived from patients diagnosed as postherpetic neuralgia in the outpatient and inpatient department of Acupuncture and Moxibustion.A total of 60 cases.According to the order of inclusion time,the patients were randomly divided into two groups according to 1:1 in random number table:treatment group A(electroacupuncture of Jiaji combined with bloodletting puncture and cupping)and treatment group B(drug group),30 cases in each group.In treatment group A,the nerve innervation area,namely the corresponding nerve segment on the ipsilateral side of the lesion and the Jiaji points on the upper and lower segments were used for acupuncture.After the gas was obtained,the electric acupuncture was connected according to the head-to-tail cut-off principle.The parameters were direct current,frequency 2 Hz,continuous wave,and current intensity 3 mA,needle remaining 30 minutes.Then use a disposable sterile syringe to puncture the pain site 5-10 times,with a depth of 1-3mm,and perform cupping at the puncture site for 3 minutes.Electroacupuncture treatment is performed once a day,and bloodletting puncture and cupping is performed every other day,and 10 days is a course of treatment,continuous treatment for 2 courses;Treatment group B received daily pregabalin capsules for analgesia and mecobalamin tablets for nutritional nerve therapy,10 days as a course of treatment,continuous treatment for 2 courses.Observe the clinical efficacy,pain visual analogue score(VAS),pain beganing to ease time,Sleep quality score(PSQI),and the changes in peripheral blood IL-6?TNF-? levels before and after treatment in the two groups of patients.Results: 1.Comparison of comprehensive efficacy between the two groups: Statistical analysis was performed at the end of the treatment.Of the 30 patients in the treatment group A,8 cases were cured,12 cases were markedly effective,9 cases were effective,and 1 case was ineffective.The total effective rate was 96.67%.Among the 30 patients in the treatment group B,3 cases were cured,6 cases were markedly effective,13 cases were effective,and 8 cases were ineffective.The total effective rate was 73.33%.The total effective rate of patients in the two groups was compared,and the difference was statistically significant,the two groups had significant effects in treating postherpetic neuralgia,but the overall clinical effect of treatment group A was better than that of treatment group B(P <0.05).2.Comparison of VAS scores between the two groups:After treatment,the VAS scores of the two groups were lower than before treatment,and the difference was statistically significant(P <0.05).The two groups of treatment methods had analgesic effect on PHN,but the treatment of patients in group A after treatment The VAS score was significantly lower than that in the treatment group B(P<0.05),indicating that electroacupuncture of Jiaji combined with bloodletting puncture and cupping for PHN was better than oral medication.3.Comparison of the Pain beganing to ease time between the two groups: the Pain beganing to ease time of the treatment group A was(5.967 ± 1.2726)days,and the Pain beganing to ease time of the treatment group B was(9.333 ± 2.2335)days.There was a significant difference in the Pain beganing to ease time between the two groups of patients after treatment(P<0.05),the difference is statistically significant,and the Pain beganing to ease time of the treatment group A is significantly shorter than the treatment group B,indicating that electroacupuncture of Jiaji combined with bloodletting puncture and cupping for PHN can play an analgesic effect earlier than oral medication.4.Comparison of sleep quality scores between the two groups: Comparing the two groups of patients before and after treatment,the sleep quality scores were lower than before treatment(P <0.05),showing that both treatment methods can improve the sleep quality of PHN patients.Comparison between the two groups after treatment,The sleep quality scores of patients in treatment group A were significantly lower than those in treatment group B(P<0.05),indicating that electroacupuncture of Jiaji combined with bloodletting puncture and cupping for PHN was superior to oral medication in improving sleep quality.5.Changes in serum IL-6 and TNF-? levels before and after treatment in the two groups:There was no significant difference in serum IL-6 and TNF-? concentrations between the two groups before treatment(P>0.05);After treatment,the concentrations of serum IL-6 and TNF-? in the two groups were lower than those before treatment(P<0.05),and the difference was statistically significant.The serum IL-6 and TNF-? levels in the treatment group A were significantly lower than those in the treatment group B(P<0.05).Conclusion: 1.Electroacupuncture of Jiaji combined with bloodletting puncture and cupping,oral drug treatment of Postherpetic neuralgia all have achieved significant clinical effects,However,electroacupuncture of Jiaji combined with bloodletting puncture and cupping have achieved significant clinical effects.2.Electroacupuncture of Jiaji combined with bloodletting puncture and cupping has a good curative effect in the treatment of postherpetic neuralgia.It can obviously alleviate pain in patients with postherpetic neuralgia,quickly exert analgesic effects,and improve patients' sleep quality.3.Electroacupuncture of Jiaji combined with bloodletting puncture and cupping can reduce the levels of serum IL-6 and TNF-?in postherpetic neuralgia patients,regulate the release of immune cytokines,reduce the stimulation of the nervous system by inflammatory reactions,and promote the recovery of damaged nerves. |
