Objective1.To investigate the resilience of lung cancer patients undergoing early chemotherapy and analyze influencing factors,so as to construct supportive-expressive group therapy study on resilience of lung cancer undergoing early chemotherapy.2.To explore the effect of supportive-expressive group intervention on resilience and quality of life of lung cancer patients undergoing early chemotherapy.Methods1.Inventory survey.From October 2018 to April 2019,lung cancer patients undergoing early chemotherapy who were hospitalized in the Department of Respiratory Medicine and Oncology in two Class A hospitals in Zhengzhou City were selected.Questionnaires were conducted to investigate resilience,psychological distress,social support,symptom distress,medical coping modes and meaning in life.Descriptive statistics,t-test,multiple linear regression were used for statistical analysis.2.Application study of supportive-expressive group therapy on resilience of lung cancer patients undergoing early chemotherapy.Supportive-expressive group therapy was formed under the guidance of literature review,investigation survey,resilience related theory,and specialist consultation and preliminary experiment were carried out to form supportive-expressive group therapy on resilience of lung cancer patients undergoing early chemotherapy.From June 2019 to January 2020,92 patients who met criteria of lung cancer patients undergoing early chemotherapy were selected for hospitalization in respiratory medicine department of a Class A hospital in Zhengzhou city.The intervention group(46 cases)and the control group(46 cases)were determined by coin toss method in the ward area.The control group received regular health education and telephone follow-up,and the intervention group carried out supportive-expressive group intervention on the basis of the control group.Resilience and quality of life were collected so as to analyzed by descriptive statistics,generalized estimation equation.Results1.Investigation survey.In this study of lung cancer undergoing early chemotherapy,Resilience score was(56.32±9.99),showed moderate and low level.Symptom burden score was(45.39±25.91),Symptom distress score was(19.28 ±10.66),Psychological distress score was(4.20±1.61),Social support score(55.52±7.21),confrontation subscale score was(17.86±3.76),avoidance subscale score was(13.56±1.58),acceptance-resignation subscale score was(9.81±2.95),Meaning in life score(78.67±12.78).Regression analysis showed that the meaning in life,symptom distress,confrontation,social support,acceptance-resignation coping,psychological distress and education level were the influencing factors of resilience,which could explain 61.0%of the total variation.2.Application study of supportive-expressive group therapy on resilience of lung cancer patients undergoing early chemotherapy.The interventions that based on a classen revised methodology were conducted.The interventions were designed to provide a supportive interactive environment,improve interpersonal relationships,promote openness and emotional expressiveness,improve coping skills,promote social and family support,integrate self-awareness,detoxify feeling around death and dying.The intervention process,86 cases including 42 cases in the control group and 44 cases in the intervention group completed intervention.(1)There was no significant difference in general demographic data and total or each dimension of resilience or quality of life between the two groups(P>0.05).(2)There were significant difference in time,group and interaction effect in total or each dimension resilience scores and total quality of life score,emotional well-being,social/family well-being and functioning well-being score(P<0.05).(3)Further simple effect analysis,The group effect of total or each dimension resilience score and total quality of life score,social/family well-being score,functioning well-being score,emotional well-being score at 1 month and 3 month after completed intervention between two groups were significant(all P<0.05).The time effect showed that,compared with before intervention,total or each dimension resilience score and total quality of life score,emotional well-being score,social/Family well-being score,functioning well-being score in the intervention group at 1 month,3month after intervention increased in intervention group(P<0.016),while the total quality of life,emotional well-being,social/Family well-being,functioning well-being of control group only increased at 1 month after intervention control group(P<0.016).Conclusions1.The resilience of lung cancer patients undergoing early chemotherapy was moderate and low level,which was affected by meaning in life,symptom distress,confrontation,social support,acceptance-resignation coping,psychological distress and education level.2.The supportive-expressive group intervention has a good effect on promoting resilience and helping to improve the quality of life of lung cancer patients undergoing early chemotherapy.
