Objective1.The comfort status scale was used to investigate the comfort status of patients with pituitary adenoma during the perioperative period.2.Apply Roy adaptive model to evaluate the influencing factors of comfort,develop comfort care measures,and build a comfort care plan based on Roy adaptive model.3.To explore the application effect of comfort nursing based on Roy adaptive mode in perioperative patients with pituitary adenoma,so as to provide theoretical guidance and evidence-based basis for perioperative nursing of pituitary adenoma patients.Methods1.This research according to the inclusion criteria,the choice of zhengzhou a certain level of first-class hospital neurosurgery in March 2018 to December 2019 proposed neural endoscopic nasal butterfly into the way of 132 cases of patients with pituitary adenoma resection,control experiment design based on clinical random number table method,66 cases of patients were randomly divided into intervention group and control group 66 cases,control group routine perioperative care,It mainly includes preoperative diet,drug guidance and examination coordination,skin preparation,preoperative body position,defecation and urination,oral and respiratory adaptability exercise,intraoperative body position nursing,postoperative body position,monitoring vital signs,complications nursing,discharge guidance,health education and other contents.The intervention group received comfort care based on the Roy adaptive model on the basis of routine care.It mainly includes preoperative improved postural exercise,oral breathing exercise,nutritional support,intraoperative temperature comfort management,postoperative postural comfort method application,nasal adaptive comfort method application,postoperative diet and nutritional support,pain nursing,oral comfort nursing and complication nursing.2.There was no significant difference between the two groups in gender,age,education,depression and anxiety,and quality of life index(P>,0.05),suggesting comparability.The patients' perioperative comfort was investigated by using the Zung anxiety self-rating scale(SAS),Zung depression self-rating scale(SDS),comfort level scale(GCQ)and Barthel index rating scale,and the comfort level,anxiety and depression score,daily living activity ability,complication rate and nursing satisfaction were compared between two groups.3.A database was established and SPSS 23.0 software was used to support statistical analysis.Mean and standard deviation,frequency and percentage were used to describe the general information of patients.The measurement data met the normal distribution,and the t test was used.When the normal distribution was not met,the kruskal-wallis H non-parametric test was used.The counting data were compared by chi-square test.Repeated measures anova was used to evaluate the variation trend of various indicators,and P<0.05 was considered as statistically significant.Results1.In this study,a total of 132 patients who underwent transsphenoidal transsphenoidal pituitary adenoma resection were enrolled.They were numbered in the order of admission and randomly divided into two groups by a number table method,with 66 patients in the experimental group and 66 patients in the control group.During the study,a total of 6 patients in the experimental group and 5 patients in the control group withdrew,with a total loss to follow-up rate of 8.33%.Finally,121 patients with transsphenoidal transsphenoidal pituitary adenoma resection were included in this study,including 60 patients in the intervention group and 61 patients in the control group.2.The differences in general information,anxiety and depression scores and daily living ability index BI between the two groups before intervention were not statistically significant and were comparable.3.There was a statistically significant difference in anxiety score between the two groups after the intervention and before the intervent:ion(P<0.05).After intervention,the difference in anxiety scores between the two groups was statistically significant(P<0.05).4.There were statistically significant differences in depression scores between the two groups after the intervention and before the intervention(P<0.05).After intervention,there was a statistically significant difference in depression scores between the two groups(P<0.05).5.After intervention,the improvement degree of life quality in the observation group was significantly better than that in the control group in psychological field,physiological field,social field,environmental field,general health status and quality of life,etc.,that is,the comfort level of the patients in the intervention group was better than that in the control group,and the difference was statistically significant(P<0.05).6.The difference of Barthel index score between the two groups after intervention and before intervention was statistically significant(P<0.05).After intervention,the Barthel index score of the two groups was compared,and the difference was statistically significant(P<0.05).7.There were 43 cases of total satisfaction in the intervention group and 7 cases in the control group.The total satisfaction in the intervention group(71.66%)was significantly higher than that in the control group(11.48%),and the difference was statistically significant(P<0.05).There were 2 cases of complications in the intervention group and 9 cases in the control group.The incidence of complications in the intervention group was significantly lower than that in the control group,and the difference was statistically significant.Conclusion1.Comfortable care of perioperative pituitary adenoma patients with Roy adaptive mode can reduce anxiety and depression of patients and improve comfort of patients.2.For patients with pituitary adenoma during perioperative period,comfortable nursing with Roy adaptive mode can improve patient satisfaction,improve daily self-care ability and reduce surgical complications.3.Comfortable nursing based on Roy adaptive mode is an effective plan for perioperative nursing of pituitary adenoma patients.
