Objective:To analyze the clinical effects of endoscopic transsphenoidal pituitary adenoma resection.Methods:Retrospective analysis of 78 patients of pituitary tumors who were performed transsphenoidal microsurgerical operation in Shanxi DaYi Hospital from March 2012 to October 2015.Collecting gender,age,endocrine test results,preoperative and postoperative imaging data,complications,pathologic results.According to the size of the tumor,we divided them into three groups,comparing resection rate between three groups;According to the degree of malignancy grade,we divided them into invasive group and non-invasive group,comparing resection rate between the two groups;In the case of PRL adenoma primary and GH adenoma primary,respectively according to the size of the tumor,We divided them into three groups,analyze the relationship between the size of tumor and the levels of postoperative hormone;In the case of PRL adenoma primary and GH adenoma primary,respectively according to the degree of tumor resection,We divided them into three groups,analyze the relationship between the the degree of tumor resection and the levels of postoperative hormone.Results:There are 78 patients in the group,including 31 males and 47 females.There are 26 cases of micro-adenoma,43 cases of large-adenomas,9 cases of gaint-adenomas;62 cases of non-invasive tumors,16 cases of invasive cancer;And the total resection in 61 cases,subtotal resection in 11 cases,most resection in 5 cases,partial resection in 1 case.Results showed that different size of tumor has the different degree of resection.the larger size of the tumor,the lower the degree of resection;different degree of invasion of tumor has the different degree of resection,The degree of resection of invasive tumor is lower than non-invasive tumor;Surgery can decrease the level of hormone,but the differet size of the tumors has the different postoperative hormone levels,the larger the tumor,the higher postoperative hormone levels;There are different level of hormone in different resection level,the higher the degree of tumor resection,the lower the level of postoperative hormone.After operation there are 6 cerebrospinal fluid leakage,10 transient diabetes insipidus,8 fever or electrolyte imbalance,2 hypopituitarism,3 nasal cavity bleeding,1 hypopsia and no death.The mean hospital stay was 11.3 days.Conclusions:Endoscopic transsphenoidal pituitary adenoma resection is the most preferred operative approach because it's safe,minimally invasiveand,effective.
