Objective:The purpose of this study was to evaluate the clinical effect of acceptance commitment therapy in middle-aged and elderly patients with post-herpetic neuralgia,with a view to improving pain acceptance,alleviating pain,improving negative mood and quality of life of middle-aged and elderly patients with post-herpetic neuralgia.Specific research objectives include:(1)To explore the characteristics and needs of psychological changes in patients with post-herpetic neuralgia in different pain duration.(2)Evaluate the clinical effect of acceptance commitment therapy.Methods:1.Applied the phenomenological research method,adopted the purpose sampling method,adopted the semi-structured interview,and interviewed 10 patients with postherpetic neuralgia mainly from the aspects of pain coping style,the needs caused by pain,and the psychological and life changes caused by pain.Colaizzi analysis method was used to analyze and sort out the data.2.The method of randomized controlled study was used to conduct convenient sampling.70 middle-aged and elderly patients with postherpetic neuralgia who were admitted to the pain department of a third-class first-class hospital in Shanxi Province were selected as the research objects.According to the random number table,they were divided into observation group and control group,35 cases each.In the observation group,35 patients were treated with acceptance and commitment therapy,while in the control group,35 patients were treated with conventional psychological nursing intervention.Before and after the intervention,assessment tools such as Chinese Chronic Pain Acceptance Questionnaire-8,Short-form McGill pain questionaire,Hospital Anxiety and Depression Scale and Short Form Health Survey Questionnaire-36 were used to evaluate and comparethe pain acceptance,pain level,anxiety and depression level and quality of life of the patients.Meanwhile,blood pressure,heart rate and sleep duration of the patients in the two groups were also compared.Results:1.Through qualitative interviews after 10 cases banded blister neuralgia patients,according to different duration of neuralgia after herpes zoster pain,divided into three stages,a total of 5 theme features extracted: when the pain lasts for 3 months or less,response to pain in the patients was helpless to endure,don't want to accept pain,strong in need of pain relief method.When the duration of pain was more than 6 months,patients had significant negative emotions such as self-perceived burden and anxiety,and their quality of life declined with the decrease of sleep duration after a long period of time,which resulted in increased demands for health education in disease knowledge and pain management.When the pain lasts for more than 1 year,the patients themselves learn some ways to deal with the pain and actively deal with the pain.2.Before the intervention,there were no statistically significant differences between the two groups in pain acceptance,pain level,anxiety and depression,life quality,blood pressure,heart rate and sleep duration(P>0.05).After the intervention,the observation group was better than the control group in pain acceptance,pain level,anxiety and depression and sleep duration,with statistically significant differences(P<0.05).In the quality of life score,the scores of the observation group in physiological function,physical pain,overall health,vitality,social function,emotional function and mental health were better than the control group,with statistically significant differences(P<0.05).Conclusion:1.The psychological characteristics and needs of patients with postherpetic neuralgia change according to the duration of the pain.At the initial stage of pain,patients mainly tolerate pain,and the most immediate need is to relieve pain as soon as possible.With the increase of pain duration,negative emotions such as feeling of suffering become an important part,and the impact of pain on quality of life gradually becomes prominent.However,the rejection of pain still exists,which makes patients have a need for health education.When the pain lasts for more than a year,the negative attitude changes and the pain is actively dealt with.It is suggested that in psychological intervention,different pain duration,the attitude of patients to deal with pain is different.At the beginning of pain,we should pay attention to guiding patients to correctly understand and accept pain and understand the needs of patients to relieve pain.In the case of negative emotions and declining quality of life,intervention can be carried out at the initial stage of pain,so that patients can actively deal with pain earlier.2.Acceptance of commitment therapy and routine psychological nursing can improve the acceptance of pain in elderly patients with post-herpetic neuralgia,reduce the pain level,and improve the anxiety,depression and quality of life caused by pain.Acceptance commitment therapy is more effective in improving pain acceptance,reducing pain,and improving pain related anxiety,depression,and quality of life(Role-Physical,Bodily Pain,General Health,Vitality,SocialFunctioning,Role-Emotional,Mental Health).Acceptance commitment therapy is simple and easy for patients to accept.It can be used as a psychological intervention therapy for clinical pain management.
