Association Of Dopaminergic Pathway Gene Polymorphisms With Postoperative Pain And Opioid Demand

Objective:To discuss the relationship between the dopaminergic pathway DRD₂rs6276,DRD₂ rs4274224 gene polymorphism and postoperative acute pain level,analgesic response,and opioid dosage,and determine whether DRD₂ can be used as a laboratory to predict postoperative pain level and opioid consumption in the population Indicators to guide perioperative pain management.Methods:In this study,150 patients underwent elective upper abdominal surgery at ASA I^?grade 18-60 years old were selected;the preoperative psychological state of the patients,the pressure pain threshold and the pain tolerance threshold were tested,and the peripheral venous blood DRD₂rs6276 and DRD₂ were detected by PCR technology rs4274224 gene polymorphism,record the degree of postoperative pain,the amount of opioid analgesia and the incidence of adverse reactions.Results:Patients with DRD₂ rs6276 A/A genotype had higher preoperative stress pain threshold and pain tolerance threshold than patients with A/G genotype and G/G genotype.Meanwhile,there was a statistically significant difference in the use of opioid drugs in rs6276 A/A genotype Learn meaning.The difference between the level of anxiety before operation and the amount of opioid analgesics used 24 hours after surgery was statistically significant.There was no significant difference in the amount of opioid analgesics used by patients with different anxiety levels 48 hours after surgery.There was no statistically significant difference in pain tolerance threshold,postoperative pain level,and incidence of adverse reactions.Conclusion:Patients with DRD₂ rs6276A/A gene have high pressure pain threshold and pain tolerance threshold,and the use of opioid analgesics is small after surgery,which can become a laboratory indicator for predicting postoperative pain and opioid consumption in the population.DRD₂ rs4274224 gene polymorphism has no obvious correlation with pain sensitivity.The dose of opioid required for postoperative anxiety patients is related to the level and time of anxiety.The patients with moderate and severe anxiety in the 24 hours after surgery have analgesic drugs Large amount of use.
