Objective:By studying the changes of liver enzymes and inflammatory factors in patients with cystic hydatid treated with hepatectomy and after the preoperative treatment with ulinastatin,our study discussed the effect of ulinastatin on liver function and the mechanism of action in patients with cystic hydatid treated with hepatectomy,so as to provide reference for clinical application.Methods:we selected 60 of cystic hepatic hydatid patients cases who underwent partial hepatectomy in the people's hospital of xinjiang uygur autonomous region from December 2017 to February 2019 and randomly divided them into the control group(group D)and the ulinastatin group(group U),with 30 cases in each group.In the U group,after anesthesia induction on the day of surgery and before skin resection,10000U/kg dose of ulinastatin was diluted into 50ml0.9% sodium chloride injection.The internal jugular vein was pumped at a speed of 50ml/h.Patients in group D received no medication.We recorded the general information of all patients(gender,age,height,weight)and the intraoperative status of all patients(operation time,hepatic portal blocking time,total infusion volume,blood loss and urine volume).Serum ALT,AST,il-6,TNF-,il-10,HMGB1 were detected at each time point including 3min before induction of anesthesia(T1),at the end of surgery(T2),at day 1(T3),day 3(T4)and day 5(T5).SPSS 20.0 statistical software was used for statistical processing of the data.Results:there was no statistical difference in gender,age,height,weight,operation time,hepatic portal blockade time,infusion volume,blood loss and urine volume between the two groups(P>0.05).There was no statistically significant difference in serum IL-6,IL-10,TNF-? and HMGB1 levels between the two groups before surgery(P>0.05).Compared with group D,serum AST and ALT levels in group U decreasedsignificantly at T3-T5(P<0.05).Serum levels of IL-6 and TNF-? decreased significantly at T4-T5(P<0.05).Serum IL-10 level increased at T3-T4(P<0.05).Serum HMGB1 content was significantly decreased at T3-T5(P<0.05).Compared with T1,serum levels of IL-6,IL-10,TNF-? and HMGB1 in both groups were increased(P<0.05).Serum AST and ALT levels increased at T2-T5(P<0.05).Conclusion:Ulinastatin can reduce the inflammatory reaction in hepatectomy of patients with cystic hydatid,thereby reducing the postoperative liver cell damage in patients and achieving effective liver protection.
