Objective:The characteristics of myocardial perfusion and myocardial mechanics in patients with hypertrophic cardiomyopathy were evaluated using real-time myocardial contrast ultrasound and two-dimensional speckle tracking technology and the relationship between the two was explored.Methods:Twenty-eight normal persons were selected as the control group and 28 patients with HCM as the case group.The case group was further divided into HCM hypertrophic segment myocardial group and HCM non-hypertrophic segment myocardial group.All subjects underwent two-dimensional echocardiography,MCE,and 2D-STI analysis to measure myocardial thickness,ejection fraction,mitral valve blood flow spectrum ratio,and mitral valve Ring velocity ratio,tricuspid regurgitation pressure difference,stroke volume,end-systolic volume,end-diastolic volume,peak intensity,curve Mean slope,area under the curve,time to peak,total longitudinal strain,total circumferential strain;the peak intensity of the thickest myocardial segment,The average slope,the area under the curve,and the time to peak were divided by the corresponding thickest myocardial thickness to obtain the normalized results.Results:Compared with the control group,PI and AUC in the HCM group decreased significantly(? =5.805,? =5.029,P<0.01),s-RS,s-PI,s-TTP and s-AUC decreased in the standardized HS group(? =7.052,? =68.416,? =13.623,? =68.256,P<0.05),s-PI and s-AUC decreased in the),NHS group(? =68.416,? =68.256,P<0.05).GLS in HS group and NHS group decreased significantly,and the difference was statistically significant(F=177.899,P<0.05).There was a negative correlation between TTP and GLS in HS group(r=0.637,P<0.001),and a positive correlation between AUC and GLS(r=0.558,r=0.688,P<0.001).In standardized HS group,there was a negative correlation between s-TTP and GLS(r=0.569,P < 0.005),and there was a positive correlation between s-PI,s-AUC and GLS(r=0.682,r=0.706,P<0.001).There was anegative correlation between TTP,s-TTP and GCS in HS group(r=0.396,r=0.441,P<0.05).Conclusion:Myocardial perfusion and myocardial mechanics were impaired in patients with HCM,and all myocardial segments were involved.The standardized myocardial perfusion parameters can better reflect the real situation of myocardial microcirculation in patients with HCM.There is a vicious circle relationship between myocardial perfusion and myocardial mechanics in patients with HCM,and the changes of myocardial microcirculation and myocardial mechanics are consistent.
