Objective: Evaluate on early-stage left ventricular and local systolic function in patients with hypertrophic cardiomyopathy caused by MYBPC3 gene by three-dimensional speckle tracing imaging(3D-STI).Methods: A total of 294 patients with HCM were diagnosed from the Department of Ultrasonography in Xijing Hospital from March 2013 to September2016.Using the method of targeting the exon capture sequencing,96 patients with HCM and the hereditary myocardium All the exons were amplified and high-throughput sequencing.39 patients with MYBPC3 gene mutations were selected by gene analysis.Forty patients were selected and the two-dimensional images were obtained by ultrasonic diagnostic apparatus(i E33)Analysis of left ventricular three-dimensional speckle tracking imaging parameters by Tom Tec software off-line analysis software.The difference of conventional two-dimensional ultrasonic parameters and three-dimensional speckle tracking imaging between the two groups was compared.Results:There was no significant difference between the data of MYBPC3 gene mutation and normal control group.There was no significant difference between left ventricular ejection fraction(p> 0.05)and left ventricular ejection fraction(p <0.05)(P <0.05),16-segment anterior wall basement,posteriorintervertebral circumference circumferential strain reduction;anterior interosseous central apical apex,anterior wall basement central apical portion,lateral wall basement central apical part,(P <0.05).The radial strain of the anterior wall base was decreased(p <0.05).The longitudinal strain of the posterior wall of the posterior wall was lower(p <0.05).Conclusion: The three-dimensional speckle tracing imaging can detect the damage of left ventricular and local systolic function in MYBPC3 gene mutations.Keywords: MYBPC3,echocardiography,three-dimensional speckle tracking imaging,cardiomyopathy,hypertrophyObjective : Evaluate on early-stage left ventricular systolic function and synchrony in hypertrophic cardiomyopathy patients with truncating mutations of MYBPC3 using three-dimensional speckle tracking imaging(3D-STI).Methods Targeted exon capture sequencing method for 274 HCM patients was completed using for exon amplification and high-throughput sequencing of 96 genes associated with hereditary cardiomyopathy and identified gene mutations.The clinical data,conventional two-dimensional echocardiography and three-dimensional speckle tracking technology of all subjects were acquired to compare the differences between the two previous groups.Results : Protein truncating mutation of MYBPC3 gene was found in 20 patients,and missense mutation was found in 19 patients.Patients with truncating mutation show severe manifestations: an early onset age,high rate of operation and sudden death in one patient.Two-dimensional echocardiographic parameters show no statistical significance between the two groups.In terms of three-dimensional speckle tracking imaging,GLS and the GRS in patients with truncating mutations show significant reduction;synchronization parameters(TLS-SD%,TRS-SD%,TLS-diff%,TRS-diff%)in patients with truncating mutations were also significantly longer(P<0.05)than the group with missense mutation.Conclusion:Three-dimensional speckle tracking imaging can be used to detect changes of left ventricular systolic function and synchrony in patients with MYBPC3 gene mutation in early stages and provide a reference for risk stratification and clinical evaluation of HCM.
