Objective:To assess the left ventricular(LV)global systolic function in patients with early stage of hypertrophic cardiomyopathy(HCM) by threedimensional speckle tracking imaging(3D-STI),and to explore the corrections among 3D strainã€Interventricular septal end-diastolic dimensionã€left ventricular outflow tract pressure gradient.Methods:A total of 90 consecutive patients with HCM were enrolled,in which 58 patients were nonobstructive HCM(HNCM),32 patients were obstructive HCM(HOCM).In addition,72 healthy subjects were recruited as a control group. An ultrasound system i E33(Philips Medical Systems,Bothell)was used to acquire two-dimensional images and analyze echocardiography parameters. Full-volume three-dimensional dynamic images were performed using matrix transducer X5-1.The LV 3D strain(3DS), longitudinal strain(LS),circumferential strain(CS), radial strain(RS) of the 16 segments were analysis using off-line Tom Tec software.GS,GLS,GRS,and GCS were averaged over the 16 segments.The differences among three groups were compared. Results:Compared with the control group,GS was no significant difference, GLS was significantly reduced,however GCS,GRS was significantly increased in HCM groups.When compared with the HNCM, GLS was significant reduced, however GCS,GRS were significant increased in the HOCM group.Bland-Altman analysis showed there were good agreements in both patients with HCM and control subjects.Conclusions:3D-STI can detect the early impairment of LV global myocardial contractility in HCM.Mainly GLS was reduced, however GCS, GRS were increased and corrected with Interventricular septal end-diastolic dimension and left ventricular outflow tract pressure gradient.3D-STI provides valuable information for clinical in early diagnosis, prevention and treatment.Objective:To assess the left ventricular(LV) regional systolic function in early stage of hypertrophic cardiomyopathy(HCM) with ventricular septal hypertrophic by three dimensional speckle tracking imaging(3D-STI),and provided a new method for evaluating LV regional systolic function in patients with early stage of HCM.Methods:A total of 70 consecutive patients with ventricular septal hypertropic asymmetry HCM were enrolled.In addition,in which 47 patients HNCM,23 patients were HOCM.68 healthy subjects were recruited as a control group.An ultrasound system i E33(Philips Medical Systems,Bothell)was used to acquire two-dimensional images and analyze echocardiography parameters.Full-volume three-dimensional dynamic images were performed using matrix transducer X5-1.The LV area strain(AS), longitudinal strain(LS),circumferential strain(CS), radial strain(RS) of the 16 segments were analysis using off-line Tom Tec software, the differences among three groups were compared.Results:Compared with the control group,LS significantly reduced in hypertrophic segments,while increased significantly or or although no statistically significant difference, but there is an increasing trend in non-hypertrophic segments or the segment corresponding to hypertrophic segments.RS significantly reduced in hypertrophic segments,while increased significantly or or although no statistically significant difference, but there is an increasing trend in non-hypertro phic segments or the segment corresponding to hypertrophic segments.CS significantly reduced in hypertrophic segments,while had no statistically differences in non-hypertrophic segment.AS significantly reduced in hypertrophic segments,while in non-hypertrophic segments or the segment corresponding to hypertrophic segments increased significantly or no statistically differences.Conclusions:The impairment of LV regional myocardial contractility existed in early stage of HCM.The main features were longitudinal and radial strain decreased in hypertrophic segments,while circumferential strain increased. 3D-STI provided a new method for evaluating LV regional systolic function in patients with early stage of HCM. |
