Meta Analysis Of The Efficacy Of Three Methods In The Treatment Of Vertebral Compression Fractures

Objective: The purpose of this meta-analysis is to compare the effects of vertebral augmentation with conservative treatment in patients with OVCF.Methods: Literatures published in Medline,Cochrane,and Embase databases were searched from 2007 to 2018 using the keywords: vertabropalsty?kyphoplasty?compression fracture?osteoporosis.Inclusion criteria were randomized controlled trials(RCTs)in which patients with osteoporotic vertebral compression fracture were treated with vertebral augmentarion(vertebroplasty or kyphoplasty)and non-operative management.Outcome measures were quality of life and pain(VAS score),function(EQ-5D).The data of included studies were calculated using the methods recommended by the Cochrane Collaboration.Systematic review and Meta-analysis was performed via RevMan 5.3 software.Analytical features and forest plots were displayed and observed.Results: A total of 6 RCTs were included,with a total of 984 patients,including 489 in the augmentation group and 495 in the conservative treatment group.Outcomes of Meta-analysis indicates: Vertebral augmentation technique provides with greater pain relief and an outstanding improvement in patients' daily function when compared with conservative treatment.The pooled data estimate indicated vertebral augmentation was associated with higher quality of life.Subgroup analysis of percutaneous vertebroplasty studies and balloon-kyphoplasty studies indicated that vertebroplasty provided better pain relief than conservative treatment but balloon-kyphoplasty did not.Conclusion: Vertebroplasty in treatment of OVCFs can achieve better pain relief when compared with kyphoplasty.Vertebral augmentation may promote the recovery of vertebral function,and effectively improve the quality of life in treatment of Osteoporotic VCFs.
