Clinical Study Of Lishi Yunpi Jiangzhuo Decoction In The Treatment Of Type 2 Diabetes Mellitus Complicated With Lipid Metabolism Disorder

Objective:Through observing the scientific research methods of self-control study before and after admission,this study studied the clinical effect of Lishi Yunpi Jiangzhuo Decoction on type 2 diabetes mellitus with spleen deficiency and dampness resistance combined with lipid metabolism disorder under the intervention of traditional Chinese medicine prescription,and provided new evidence for Lishi Yunpi Jiangzhuo Decoction in treating type 2 diabetes mellitus with lipid metabolism disorder of spleen deficiency and dam It is hoped that this study will be used as an opportunity to further explore the new ideas and methods of TCM in the treatment of type 2 diabetes mellitus with lipid metabolism disorder;to systematically observe the changes of various laboratory indicators in patients with type 2 diabetes mellitus complicated with lipid metabolism disorder by Lishi Yunpi Jiangzhuo Decoction,the specific indicators are as follows:blood sugar control Assessment indicators included fasting blood glucose(FBG),2-hour postprandial b l ood glucose(2hBG),glycosylated hemoglobin(HhAlc);assessment indicators of blood lipid control included triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C);safety monitoring indicators were the main.In order to evaluate the liver function before and after treatment,including alanine ami notransferase(ALT),aspartate aminotransferase(AST),glutamyltransferase(GGT);in addition,it is necessary to observe the changes of TCM syndrome score hefore and after treatment to evaluate the treatment of patients' subjective symptoms;on the basis of clinical observation,try to explore Li Shiyunpi.The possible mechanism of Jiangzhuo Formula in treating type 2 diabetes mellitus with lipid metabolism disorder provides ideas for further research.Method:In this study,60 cases of type 2 diabetes mellitus with lipid metabolism disorder aged 30-70 years in Shenzhen Hospital of Guangzhou University of Traditional Chinese Medicine from April to October 2018 were randomly divided into two groups,30 cases in each group,after strict screening of inclusion and exclusion criteria.The control group was treated with routine western medicine(hypoglycemic,lipid-lowering).Atorvastatin calcium tablets were administered with antilipid drugs at 10 mg,once before bedtime.On the basis of the control group,the observation group was treated with Lishi Yunpi Jiangzhuo Decoction(Dangshen 30g,Poria Cocos 20g,Hongqu 9g,Atractylodes lanceolata 15g,roasted licorice 5g,Scutellaria baicalensis 10g,Coptis chinensis 10g,dried ginger 10g,Chaihu 10g,Pueraria 30g,Red Paeonia 20g,Xianling spleen 30g).During the treatment observation period,if the patients in the group had dry stool,dry mouth and throat,thirst and other negative symptoms such as deficiency of Yin score,then add Zhimu 10 grams,trichosanthin 20 grams,cooked rhubarb 10 grams,Xuanshen 15 grams.Patients with tongue coating thick and greasy,loose stool,abdominal distension discomfort,dry mouth,bad breath and other intestinal manifestations of dampness and heat are added with 10 grams of Ruxiang,10 grams of Magnolia officinalis,30 grams of raw coix,10 grams of Peilan.Treatment.The observation period was February.The changes of laboratory indexes and TCM symptoms before and after treatment were compared between the two groups after 2 months.The laboratory indexes included FBG,2hBG,HbAlc,TG,TC,LDL-C,HDL-C,ALT,AST and GGT.To explore the interventional effect and therapeutic value of Lishi Yunpi Jiangzhuo Decoction in the treatment of type 2 diabetes mellitus complicated with lipid metabolism disorder,to evaluate its safety and practicability,and to preliminarily explore its possible mechanism of action,so as to provide new data support for follow-up studies.Result:There were 60 patients who met the inclusion criteria,3 patients who were excluded from the study,and a total of 57 patients,including 28 in the observation group and 29.in the control group.The results of baseline analysis showed that there was no significant difference between the two groups in general conditions before treatment,such as gender,age,course of disease,TCM syndrome score,blood sugar,blood lipid,liver function,etc.The experimental data were objective and true,and there was comparability.The results showed that:(1)there were significant differences in TCM symptom score,blood sugar and blood lipid between the observation group and the control group before and after treatment,and the difference was statistically significant(P<0.05);(2)TC was significantly different between the observation group and the control group after treatment(P<0.05).(3)There were significant differences in TCM syndromes,blood sugar and lipid metabolism disorders between t.he observation group and the control group(P<0.05).(4)Safety indicators:Liver function:ALT,AST and GGT in the observation group were within the normal range before and after treatment.There was no significant difference between ALT and GGT in the control group before and after treatment(P>0.05).After treatment,ALT was significantly higher than before treatment,the difference was significant(P<0.05).Adverse reactions:2 cases in the control group had constipation symptoms in the course of treatment,which belonged to mild gastrointestinal adverse reactions,and did not need any treatment.The constipation symptoms could be alleviated by themselves after the withdrawal of foot therapy.No adverse reactions occurred in the observation group during the course of treatment,and no gastrointestinal discomfort was observed.There was no abnormal liver function in the two groups before and after treatment,and no patient quit the clinical study because of abnormal liver function,suggesting that the two groups of patients had good safety in the course of treatment.In addition,the results of routine hematuria,electrocardiogram,renal function and other general monitoring indicators before and after treatment in both groups were within the normal range,without any abnormalities.Conclusion:This study suggests that the combination of Western medicine and Lishi Yunpi Jiangzhuo Decoction is superior to western medicine alone in the treatment of type 2 diabetes mellitus with lipid metabolism disorder(spleen deficiency and dampness obstruction type).It can not only improve the symptom score of patients,but also has a clear effect of hypoglycemia and hypolipidemia.It is safe and has no effect on liver function.Side effects and adverse reactions.This study provides a scientific basis for the treatment of type 2 diabetes mellitus with lipid metabolism disorder(spleen deficiency and dampness resistance type)by Lishi Yunpi Jiangzhuo Decoction,and lays a theoretical and research foundation for the further study of Lishi Yunpi Jiangzhuo Decoction.
