ObjectiveThe metabolic pathways of rats with post-operative fatigue syndrome were analyzed by means of metabolomic analysis.The pathological mechanism of post-operative fatigue syndrome and the effect of Jiangzhuo Shengqing Granule on improving post-operative fatigue were discussed from the level of metabolomics.Pre-mechanism provides laboratory basis for clinical application and further development of Jiangzhuo Shengqing Granule and new ideas and means for prevention and treatment of POFS.MethodsThirty adult healthy SPF SD rats were randomly divided into model group(group A),administration group(group B)and normal group(group C).Rats in group A and B were given mid-intestinal resection kiss and POFS model.Rats in group C were anesthetized with chloral hydrate without operation.Jiangzhuo Shengqing Granules(dafupi,huomaren,renshen,huangqi,wuweizi)were prepared with distilled water at a concentration of 0.32g/ml.The solution was stored in a refrigerator at 4 C and heated to room temperature by water bath before gavage.From the first day after the operation,rats in group B were given Jiangzhuo Shengqing granule solution by gastric perfusion with dosage of 2.0ml/100g,once in the morning and afternoon.Rats in group B and C were given distilled water of the same volume at the same time,lasting until the seventh day after the operation,and abdominal aorta blood samples were collected on the eighth day after the operation.The plasma sample was thawed on ice.The sample was extracted with 50%methanol pre-cooled at 120 ml for 1 minute and incubated at room temperature for 10 minutes.Then the extract mixture was stored overnight at-20 C.The plasma sample was centrifuged at 4000r/min for 20 minutes.The parameters of liquid chromatography and mass spectrometry were set.High performance liquid chromatography tandem high resolution mass spectrometer TripleTOF 5600 was used for positive and negative separation.The metabolome of plasma samples(including QC samples)was detected in sub-mode.The LC-MS original data file was converted into mzXML format.Then XCMS,CAMERA and metaX toolbox were processed by R software to screen the substances quantitatively and differentially.ResultsTwenty-three plasma samples were obtained,of which the number of plasma samples in group A was n=7,that in group B was n=6,and that in group C was n=10.The total number of substances extracted by XCMS software was 6828.There were significant differences in tryptophan metabolism,pentose and glucuronic acid conversion,arginine and proline metabolism,cytochrome P450 on the metabolism of exogenous compounds,porphyrin and chlorophyll,folic acid synthesis,vitamin B6,linoleic acid metabolism and other metabolic pathways between group A and group B(P<0.05).There were significant differences in metabolic pathways such as juice secretion,peroxisome proliferator activated receptor(PPAR)signaling pathway and linoleic acid(P<0.05).Compared with group C,there were significant differences in metabolic pathways such as linoleic acid,arginine and proline,cytochrome P450,caffeine,porphyrin and chlorophyll metabolism in group B.The difference was significant(P<0.05).ConclusionPostoperative fatigue in rats with midgut resection is associated with increased 5-HT synthesis,decreased blood circulation and energy supply disturbance caused by changes in metabolic pathways such as tryptophan,arginine,pentose and glucuronic acid.Jiangzhuo Shengqing Granule can improve postoperative fatigue by regulating bile secretion and peroxisome proliferator activated receptor(PPAR)signaling pathway.Because the identification of non-target metabolites is still insufficient,the qualitative and quantitative analysis of specific metabolites still needs further experimental study.
