ObjectiveTo discuss the security and effectiveness of different ways of blood transfusion using in total hip replacement.Methods70 cases of patients with total hip replacement were randomly divided into autologous blood transfusion group and allogeneic blood transfusion group(n=35).The patients in allogeneic group only receive allogeneic transfusion.The patients in autologous group predeposite autologous whole blood 400 ml before operation.They also transfuse intraoperative salvage of shed blood.Before surgery(T1),one day after surgery(T2),Three days after surgery(T3),collected and analyzed the data that are the level of WBC,RBC,Hb,Hct,Plt,PT,APTT,Fib.And also Plasma IL-6,IL-8,TNF-α were measured by ELISA.ResultsIn the autologous blood transfusion group,indicators after autologous blood transfusion like RBC、Hb、Hct、WBC significantly improved at T2 and T3.However indicators like coagulation,Plt and Fib significantly declined at T2 and T3 compared with those before transfusion,while PT,APTT significantly increased(P<0.05).In the allogeneic blood transfusion group,the changes of coagulation function indexes were basically consistent with autologous blood transfusion group.IL-6 of allogeneic blood transfusion group were significantly higher at T3 than at T1;at T2 and T3,IL-8 and TNF-α were significantly higher than at T1(P<0.05).At T2 and T3,IL-8 and TNF-α in autologous blood transfusion group were significaly lower than allogeneic blood transfusion group at the same period(P<0.05).ConclusionThere was no difference in the effect of autologous or allogeneic blood transfusion on the coagulation.Predeposited autologous whole blood associating with intraoperative autologous blood can reduce the usage amount of allogeneic blood and save blood resources,and may also avoid excessive inflammatory response and reduce the degradation of extracellular matrix,contribute to post operative recovery.
