ObjectiveTo study the rehabilitation effect of cochlear implantation and its influence factors.MethodsA total of 295 patients with severe sensorineural hearing loss who were treated with CI in Department of Otorhinolaryngology of Qianfoshan Hospital,Shandong Province,they were studied according to whether they wore hearing aids,residual hearing,secretory otitis media and Waardenburg syndrome before operation.White matter lesion,degree of inner ear malformation,age at the time of implantation,operative method and rehabilit-ation time were single influencing factors.Categories of auditory perf was used.Speech intelligibility rating(SIR)?speech intelligibility(CAP)and video and recording analysis were followed up to collect auditory speech grade data at 1,2 and 3 years after CI.The single factor rank sum test and multivariate Logistic regression analysis were performed with SPASS16.0 software to obtain the effect of each factor on the recovery of hearing and speech after CI operation.ResultsExcept for 2 patients with small cochlear nerve combined with stenosis of the internal auditory canal,the threshold was 45 dB,and the average threshold of sound field was 25-40 dB.The single factor rank sum test results show that:hearing aids(P=0.0213)?age of implantation(P=0.0491)?rehabilitation time(P=0.0385)?residual hearing(P=0.0431)?inner ear malformation(P=0.0281)?white matter lesion of the brain(P=0.0289)have significant difference in CAP;hearing aids(P=0.0362)?age of implantation(P=0.00751)?rehabilitation time(P=0.0269)?residual hear(P=0.0307)?inner ear malformation(P=0.0128)?white matter lesion of the brain(P=0.0134)have significant difference in CAP.Surgical methods(P=0.645)?tympanitis(P=0.932)and Waardenburg syndrome(P=0.596)have no statistical significance to the difference of CAP,Surgical methods(P=1.014)?tympanitis(P=1.540)?Waardenburg syndrome(P=0.725)have no statistical significance to the difference of SIR.Multiple factor Logistic regression analysis showed that:length of post implantation(x~2=18.693,P<0.0001)?white matter lesion of the brain(x~2=7.591,P<0.0001)have significant correlation with CAP,length of post implantation(x~2=26.302,P<0.0001)?white matter lesion of the brain(x~2=14.196,P<0.0001)have significant correlation with SIR.ConclusionMost of the severe/very severe sensorineural sense of a certain period of time in patients after cochlear implantation,hearing and speech level has greatly improved,and the rehabilitation effect is influenced by many factors.Whether to wear hearing aids and the length of time before surgery,preoperative residual hearing,Mondini malformation,brain white mass lesions,cochlear nerve dysplasia,age at implantation,rehabilitation time after implantation is one of the important factors affecting the rehabilitation efficacy after surgery.The surgical method,secretory otitis media,Waardenburg syndrome does not affect cochlear implantation after hearing speech rehabilitation.
