Objiective:To valuate the hearing and speech rehabilitation efficacy in prelingually deaf children after cochlear implants and the related influencing factors analysis.Methods:Eighty-five cochlear implantees were involved in this investigation; interviewed the implantees’ parents by telephone. To valuate the hearing and speech rehabilitation efficacy in prelingually deaf children according to Categories of Auditory Performance (CAP) and Speech Intelligibility Rating(SIR); then analyzed the effect of CAP/SIR rating results and its possible influence factors including cerebral white matter, time, age, gender and income by using single factor chi-square test and rank in the group and test and the multi-factor Logistic regression analysis and principal component analysis.Results:Principal component analysis:Cerebral white matter most affect factors on the CAP and SIR, followed by time factor, age, gender and family income.Single factor analysis:Presence of cerebral white matter abnormalities in the auditory center (P= 0.013), presence of cerebral white matter abnormalities in the speech center (P= 0.004), implanted period (P= 0.007) is associated with CAP;cerebral white matter abnormalities (P= 0.004), presence of cerebral white matter abnormalities in the auditory center (P= 0.001), presence of cerebral white matter abnormalities in the speech center (P< 0.001), implanted period (P= 0.012) is associated with SIR;gender, family economic conditions, implanted age,modulated age,language training time after surgery has nothing to do with CAP and SIR.Multiariable Logistic regression analysis:family economic conditions (P=0.047) have significant correlation with CAP,the higher family income with the higher CAP score, the CAP score of high family income is 2.38 times of low family income.family income (P= 0.029), implanted period (P= 0.005) is associated with SIR;family income and implanted period is the protection factor of SIR,the higher family income and the longer the time after implantation with the higher SIR score, the SIR score of high family income is 3.33 times of low family income,the SIR score of the longer time after implantation is 4.63 times of short time after implantation.Conclusions:Cerebral white matter abnormalities,presence of cerebral white matter abnormalities in the auditory center and speech center are most affected the hearing and speech rehabilitation efficacy.Cochlear implant the earlier age, the hearing and speech rehabilitation efficacy is better;The longer the time after the cochlear implant, the hearing and speech rehabilitation efficacy is better; High family income is the protection factor of the hearing and speech rehabilitation efficacy, but not the determining factor,the postoperative long-term effective training to maximize the effect of rehabilitation.
