Objectives:We aimed to demonstrate the effectiveness of Xinmailong on short-term hemodynamics,clinical effect and cardiac function of patients with acute decompensated chronic heart failure when combined with guideline recommend therapy.Methods:Our study was a randomized,open-label,controlled trial.Patients admitted to First Affiliated Hospital of Kunming Medical University for acute decompensated chronic heart failure who met the including criteria were randomly assigned to either Xinmailong+guideline recommend therapy or guideline recommend therapy arm.Xinmailong was administrated(5 mg/kg twice daily)with an interval more than 6 hours for 7days2.Echocardiography,electrocardiogram and chest radiograph were performed at admission and discharge.Complete blood count,liver and renal founction,blood gas analysis were performed according to the protocol.Self reported score of symptoms were recorded everyday as well as the vital signs.3.The data were analyzed by SPSS 17.0 statistical software package,and the difference was significant at two-tail p<0.05.Results:1.From January 2018 to March 2019,40 patients with acute decompensated chronic heart failure patients were enrolled(Xinmailong vs guideline recommend therapy:24 vs 16).Baseline characteristics were balanced between the two arms.2.The improvement of dyspnea,fatigue,anorexia and edema in patients with acute decompensated chronic heart failure in Xinmaillong arm were more obvious than control arm.3.Xinmailong showed trend of more improvement in changes of LVEDV(-19.73±26.91 vs-2±15.05,ml,p=0.096),LVESV(-25.36±28.47vs-4.67±15.84,ml,p=0.119),LVEF(6.00±7.80 vs 1.9±5.31,%,p=0.176),FS(5.31±7.69 vs 0.79±4.77,%,p=0.067),compared to control.There were no statistical differences in the diastolic function parameters between the two arms.4.There was no significant difference in the amount of loop diuretic(Xinmailong vs guideline recommend therapy,591.77±527.88 vs 788.24±899.17,mg,p=0.398),and so do other intravenous medications(p>0.05)during the hospitalization period.A trend of more urine volume were observed in Xinmailong group.5.Blood gas showed higher in carbon dioxide partial pressure in Xinmailong gruop on the fifth day.There was no significant difference in BNP,liver and kidney function,myocardial injury biomarker,during hospitalization days.Conclusions:Xinmailong can significantly alleviate the symptoms of patients with acute decompensation ofchronic heart failure,showing the trend of enforcing myocardial contraction,decreasing volume of left venticle,and enhancing diuresis.So it could be a choice for patients with acute decompensated chronic heart failure.
