Objective(s):In order to better meet clinical needs,this topic mainly explores the relationship between preoperative diagnosis of non--traumatic hip disease and the effect of total hip arthroplasty,and exploring whether the age of total hip arthroplasty in non--traumatic hip disease is advanced,and analyzing risk factors.Methods:Collected 2,456 cases of artificial hip joint replacement from January 2016 to May 2018,the 92nd Hospital of the People,s Liberation Army Joint Service Support Force.The subject of this study was artificial total hip arthroplasty for the first non-traumatic hip disease,through conditional screening(removal of fresh,old hip fracture cases,previous hip surgery history cases,half-hip replacement cases,revision cases),a total of 667 cases were selected according to the research criteria of this subject.Corrected the preoperative diagnosis of inaccurate cases,the correction was based on preoperative hip imaging,postoperative diagnosis,medical history,postoperative pathological diagnosis,and the final diagnosis was made,and the diagnostic correction rate was calculated.Statistical sample data,including the number of people in different age groups,the number of males and females,and the number of hip patients were statistically analyzed,and the corresponding statistical charts were obtained.Preoperative diagnosis of correct cases of 49 cases(group I)and revised cases of 49 eases(group 2),there was no signifieant difference in preoperative Harris pain and functional score between the two groups.Therefore,comparative analysis of the difference in Harris pain and functional scores after 1 year of follow-up.At the same time,the X-ray imaging of the hip was analyzed 1 year after operation.The indexes included prosthesis loosening,abnormal acetabular abduction angle,abnormal angle between prosthesis and medullary cavity,and statistieal analysis of whether there is a difference between the two groups.Finally,the past history of the sample cases was statistically analyzed,and the risk factors related to the early age of the initial total hip arthroplasty for non-traumatic hip disease were obtained.Results:1.The average age of total hip arthroplasty in the whole sample was 54.08 years old,and the proportion of middle-aged and middle-aged people exceeds 50%,(50,60),accounting for 31.18%,(40,50),accounting for 22.34%;2.There was a statistically significant difference between the preoperative diagnosis wrong group and the preoperative diagnosis correct group(Harris pain score:?2=1 7.507,P<0.001;Harris function score:?2=16.217,P<0.001),there was a statistically significant difference between the two groups in the 1 year postoperative imaging poor index(Z=-7.482,P<0.001);3.Taking hormones and drinking heavily was the main risk factor for early age of total hip arthroplasty(Logistic regression analysis,P<0.001);4.The total sample of total hip arthroplasty for non-traunatic hip disease was higher inmales than in females.The difference was statistically significant(?2=117.165,P<0.001).There was no difference in surgical site(?2=0.433,P=0.510);5.Ischemic necrosis of the femoral head was the most common in non-traumatic hip disease,accounting for 76.31;male incidence was significantly higher than female,accounting for 64.83%,the difference was statistically significant(?2=44.796,P<0.001);the left and right ratios were 49.51%and 50.49%,respectively,and there was no statistical difference(?2=0.049,P=0.825);6.The overall correction rate of the sample is 7.35%,the lowest correction rate is ischemic necrosis of the femoral head,accounting for 5.11%,and the highest correction rate is tuberculosis of hip joint,accounting for 50%.Conclusion(s):1.Inaccurate preoperative diagnosis is one of the reasons for poor efficacy and low satisfaction after total hip arthroplasty.2.The age of total hip arthroplasty in non-traumatic hip disease is significantly advanced.Taking corticosteroids and drinking heavily is the main risk factor for total hip arthroplasty rejuvenation.Gout,diabetes,and smoking are related to the rejuvenation of total hip arthroplasty.
