Research background:Diabetes mellitus has become one of the major chronic non-communicable diseases that endanger the health of residents.According to the international diabetes federation,there were 387 million diabetic patients in the world in 2014,of which about 90%were type 2 diabetic patients.In 2014,there were 96.29 million diabetic patients in China.As the number of diabetes deaths and direct medical costs increase,the burden of diabetes disease becomes increasingly heavy.Since the implementation of chronic disease monitoring in Kunshan,the incidence of type 2 diabetes has been increasing year by year.Objective:To understand the incidence trend of type 2 diabetes mellitus in Kunshan from 2008 to 2016.To explore the influence of gender and age on the development of type 2 diabetes mellitus.It provides scientific basis for proposing reasonable prevention and treatment measures to reduce the occurrence of type 2 diabetes and the disease burden of type 2 diabetes.Methods:The monitoring data of diabetes in Kunshan were collected,and the registered cases of type 2 diabetes with the diagnosis date of 1 solstice in January 2008 and 31 December 2016 were screened out.According to the classification of the international disease code(ICD-10),the ICD-10 code for type 2 diabetes is E11.The diagnostic criteria for type 2 diabetes cases reported were in accordance with the diagnostic criteria for diabetes published by WHO in 1999:fasting plasma glucose level was greater than or equal to 7.0mmol/L;In the glucose load experiment,the 2-hour blood glucose value was greater than or equal to 11.1mmol/L.The selected type 2 diabetes cases were divided into the whole population,male population,female population,people under 60 years old and people over 60 years old according to the purpose of the study and the influencing factors of diabetes mellitus to be studied:gender and age(60 years old).The crude incidence(crude incidence=the number of patients in a certain year/the average population in the current year)was calculated for each population in different years,and the standardized incidence was calculated based on the population structure of the 2010 national census.The logarithmic linear regression model was used to estimate the Annual Percentage Change(APC),which was used to evaluate the time trend of incidence.Results:1.The number of type 2 diabetes patients in Kunshan from 2008 to 2016 was 22,545.There was a positive correlation between the number of group diseases and age in the age group of "20 years old-" to "60 years old-",and there was a significant increase from the age group of "40 years old-".The crude incidence and standardized incidence of type 2 diabetes in the whole population showed an increasing trend from 2008 to 2014,and a decreasing trend from 2014 to 2016.2.From 2008 to 2016,the incidence of type 2 diabetes was 10,986 among males and 11,559 among females in Kunshan.In the age group of "20 years old-" to "50 years old-",the number of male patients was higher than that of female patients.In the age group of "55-" and above,the number of female patients was higher than that of male patients.The crude incidence rate and standardized incidence rate of male population and female population are consistent with each other,showing an upward trend from 2008 to 2014 and a downward trend from 2014 to 2016.Except in 2016,the crude incidence and standardized incidence of female population were higher than that of male population.3.From 2008 to 2016,the incidence of type 2 diabetes in kunshan city was 10029 in people under 60 years old and 12516 in people over 60 years old.In all years,the number of patients aged 60 and over was higher than that of those aged under 60.There was no significant increase or decrease in crude incidence and standardized incidence in the population under 60 years old.The crude incidence and standardized incidence of people aged 60 and above showed an increasing trend from 2008 to 2014,and a decreasing trend from 2014 to 2016.The crude morbidity and standardized morbidity of people aged 60 and above were significantly higher than those aged under 60.Conclusions:1.The incidence of type 2 diabetes in Kunshan showed an increasing trend from 2008 to 2014 and a decreasing trend from 2014 to 2016.VVhether the declining trend will continue in the following years remains to be determined and verified by follow-up monitoring data.2.Gender has an impact on the occurrence of type 2 diabetes,but the evidence supporting the results of this study is not very sufficient,and further research is needed.3.Advanced age significantly increases the risk of type 2 diabetes,and the prevention and treatment of type 2 diabetes should focus on middle-aged and elderly people.
