Objective:1. Through retrospective study clinical data of different hospital times in type 2 diabetes patients, analysis the incidence rate of diabetic nephropathy in type 2 diabetes patients.2. Analysis the risk factors of diabetic nephropathy in type 2 diabetes.Methods:1. The research object and grouping:152 cases type 2 diabetes patients stay in our endocrine metabolism, endocrinology and elderly endocrinologist many times. In the above departments of hospitalized patients with complications of materials as early as the length of time as the starting time of observation. The corresponding object as into the group. Again in interval of 5 years in the department in hospital patients with complete complication data into groups for five years. Again in interval of 10 years hospitalized in the department has a set of complete data of patients with complications included in 10 years. There are 57 patients both into 5 years group, and conform to the group in 10 years. The last time the length of time as the end of the observation time, the corresponding object as out of group.2. All the research object in general:152 cases (male 93 cases, female 59 cases), into the group the age 29-91 year,the average (58.0±12.9) year, the course of diabetes 0-32 year, the average 5.0 (1.0,10.0) year.3.5 year group:120 cases(male 67 cases,female 53 cases), into the group the age 29-91 year, the average (58.4±13.1) year, the course of diabetes 0-32 year, the average 5.0 (1.8,10.0) year.4.10 year group:89 cases(male 59cases,female 30cases), into the group the age 29-84 year, the average (57.0±12.4) year, the course of diabetes 0-32 year,the average 4.0 (2.0,8.8) year.5. Collect and record all the research object the clinical data of hospitalized at a time (gender, age, duration of diabetes, previous medical history, measuring blood pressure, height, weight, BMI calculation) and biochemical examination: blood lipid (TG、TC、HDL-C、LDL-C), blood glucose(FBG, PBG2h), glycated hemoglobin (HBALC)、renal function (UA、Cr),24h urine trace albumin (24hU-Alb)、24h urinary albumin (24hU-Pro), fundus examination,routine ECG examination, Abdominal ultrasound and Chest X-ray examination.6. Statistical process:① all study object observation start into the group and the end of the observation group, the prevalence of type 2 diabetes mellitus and diabetic nephropathy. ② 5 years group of type 2 diabetes mellitus and diabetic kidney disease prevalence and incidence of a disease. ③ 10 years group of type 2 diabetes mellitus and diabetic kidney disease prevalence and incidence of a disease. ④ Through the logistics regression analysis risk factor for type 2 diabetes mellitus and diabetic nephropathy.7. Using SPSS 16.0 statistical software for data analysis, test the index distribution, normal distribution data to mean ± standard deviation (x ± S) display; Non-normal distribution data with median (four digits) display; Mean differences between the two groups to compare normal distribution by using two independent sample t test mean; Non-normal distribution mean differences between the two groups compared with rank and inspection; The comparison of rate using x2 test; The single factor correlation analysis uses Spearman correlation analysis; Multiple factors analysis using logistics regression analysis; P<0.05 difference was statistically significant (P<0.05).Results:1. All the objects into groups, the prevalence of type 2 diabetes mellitus and diabetic nephropathy was 30.9% (47/152); Watch over the group, the prevalence of type 2 diabetes mellitus and diabetic nephropathy (77/152), 50.6% higher compared with the group (P=0.000), the difference was statistically significant.2.5 years group five years ago the prevalence of type 2 diabetes mellitus and diabetic nephropathy was 29.1% (35/120); Observed after 5 years the prevalence of type 2 diabetes mellitus and diabetic nephropathy was 42.5% (51/120), higher compared with five years ago (P=0.031), the difference was statistically significant3.10 years group 10 years ago, the prevalence of type 2 diabetes mellitus and diabetic nephropathy was 32.5% (29/89); After 10 years of observing the prevalence of type 2 diabetes mellitus and diabetic nephropathy was 53.9% (48/89), significantly increased from 10 years ago (P=0.004), the difference was statistically significant. 4.5 years merger diabetic nephropathy in type 2 diabetes incidence was 18.8%(16/85),10 years the incidence of diabetic nephropathy merger was 31.6%(19/60).5. Duration of diabetes, high blood pressure, uric acid and diabetic nephropathy were positively correlated (P<0.05), regression analysis, high blood pressure, uric acid, duration of diabetes is a risk factor for diabetic nephropathy (P<0.05).Conclusion:1. Type 2 diabetes patients with the increase of course, combined prevalence of diabetic nephropathy.2. Type 2 diabetes patients merge the incidence of diabetic nephropathy was 18.8% for five years.3. Type 2 diabetes patients merge the incidence of diabetic nephropathy was 31.6% for ten years;4. Duration of diabetes, high blood pressure, uric acid is a risk factor for diabetic nephropathy.
