A Case-control Study On Risk Factors Of Hypertension In Gannan Tibetans

Objective:To explore the risk factors of hypertension in Gannan Tibetans,and to make a good foundation for the prevention and control of hypertension here.Method: The investigation was carried out in Xiahe County and Hezuo County,Gannan Tibetan Autonomous Prefecture,Gansu Province,by case-control study.A total of 1449 cases of Tibetan adult essential hypertension and non-hypertensive patients were randomly selected from the two counties,and questionnaires,medical examinations and laboratory tests were carried out.A total of 1310 cases of complete information were obtained,the effective rate was 90.4%,including 655 cases in the case group and the control group.Through the statistical analysis of the collected data by SPSS22.0,logistic regression analysis was used to screen out the main risk factors of hypertension in the Gannan Tibetans.Result:1.Compared the general situation and relevant measurement indexes of case group and control group,the results showed that age,diastolic blood pressure,systolic blood pressure,height,weight,waist and hip circumference,BMI,daily salt intake,TG and TC were all higher in the case group than in the control group,with statistically significant differences(P<0.05).HDL-C was lower than the control group,and the difference was statistically significant(P<0.05).2.The results of multi-factor Logistic regression analysis showed that statistically significant factors affecting hypertension were:(1)Demographic characteristics: age [OR=1.135,95% CI(1.110,1.161)],high school education [OR=5.405,95% CI(1.741,16.780)],college degree or above[OR=14.985,95% CI(4.265,52.655)],family monthly income 1000-2999 Yuan[OR=11.473,95% CI(6.721,19.584)],family monthly income 3000-4999 yuan[OR=2.683,95% CI(1.368,5.263)],family monthly income> 5000 yuan[OR=220.851,95% CI(57.793,843.963)] were risk factors for hypertension;(2)Behavioral Characteristics: smoking [OR=3.172,95% CI(1.818,5.53)],drinking [OR=3.249,95% CI(1.686,6.261)],Salt > 6 g/day [OR=6.385,95% CI(3.389,12.027)] were risk factors for hypertension.(3)Human measurement indicators and biochemical indicators:salt sensitivity[OR=3.198,95%CI(2.003,6.645)],central obesity [OR=3.854,95%CI(2.235,6.645)],waist circumference increase [OR=1.010,95%CI(0.991,1.030)],overweight [OR=3.131,95%CI(2.008,4.833)],obesity [OR=17.858,95%CI(9.966,32.001)] were risk factors for hypertension.(4)Eating habits: staple food 250-350 g /day [OR=25.443,95% CI(2.929,221.017)],staple food 400-500g/day [OR=68.979,95% CI(8.302,573.126)],staple food>500g/day [OR=9.365,95% CI(1.164,75.325)],red meat>500g/week[OR=15.448,95% CI(5.048,47.277)] were risk factors for hypertension;milk and dairy products 500-900g/week [OR=0.298,95% CI(0.129,0.690)],milk and dairy products 500-900g/week [OR=0.139,95% CI(0.060,0.322)],milk and dairy products?1kg/week [OR=0.089,95% CI(0.038,0.210)],white meat 150-250g/week[OR=0.190,95% CI(0.082,0.440)],white meat 300-500 g/week [OR=0.124,95%CI(0.016,0.980)],white meat>500g/week [OR=0.022,95% CI(0.001,0.939)],1-2eggs/week[OR=0.134,95% CI(0.057,0.316)],3-5 eggs/week [OR=0.020,95% CI(0.004,0.090)],fresh vegetables [OR=0.301,95% CI(0.133,0.682)] were protective factors for hypertension.Conclusion:1.Age,education level,household income,smoking,drinking,obesity,excess salt,salt sensitivity,staple food and excessive intake of red meat were risk factors for hypertension in Gannan Tibetans;milk and dairy products,ingestion of eggs,chicken,duck meat and fresh vegetables were protective factors for hypertension.2.According to the results of this study,the following suggestions were proposed for the prevention and control of hypertension in the Gannan Tibetans:health education,promote healthy lifestyles,build a reasonable diet,keep a healthy weight,and plan interventions for high-risk groups.
