Objective: Through comparative analysis of NPWT combined with peracetic acid solution and aseptic saline irrigation in the treatment of stage III/IV pressure injury patients,wound contraction rate,stealth depth,EGF content,related inflammatory indicators(WBC,CRP,PCT,Changes in IL-6)and length of stay,etc.To explore the difference of curative effect between the two methods,and to provide a more safe,effective,economical and practical clinical treatment for chronic refractory wounds such as stage III/IV pressure injury.Methods: The patients with stage III/IV stress injury hospitalized in the Department of Burn,Gansu Provincial people's Hospital from December 2016 to November 2018 were randomly divided into experimental group and control group according to the inclusion and exclusion criteria after one week of basic treatment.The experimental group was washed with NPWT combined with peracetic acid solution and the control group was washed with NPWT combined with aseptic normal saline.both groups were treated according to NPWT operation process.the negative pressure value was adjusted to-125 mmHg.the NPWT device was replaced every 7 days and intervened for 14 days.If blockage and poor drainage occur during NPWT treatment,NPWT drainage device should be replaced in time.The related indexes of the two groups on the 0th,7th and 14 th day were recorded respectively.on the 0th and 14 th day,the granulation of the relatively clean wound was stored in the refrigerator at-80 ?,and the quantitative detection of EGF was performed at the later stage.Collect and collate the data,carry on the average statistical processing.All the data were analyzed by statistical software SPSS 20.0.Results:1.There was no significant difference in age,sex and corresponding wound indexes between the experimental group and the control group before intervention,and there was no significant difference in inflammatory indexes including WBC,CRP,PCT,IL-6 and EGF content between the two groups before intervention.The significance(P > 0.05),that is to say,there was comparability between the two groups of subjects;2.After 14 days of intervention,there were significant differences in wound contraction rate,latent depth,WBC,CRP,PCT,IL-6,EGF and hospitalization time between the experimental group and the control group(P < 0.01).Conclusion:1.Peracetic acid combined with NPWT in the treatment of stage III/IV pressure injury can significantly reduce inflammatory indexes,increase wound EGF content,promote wound healing and shorten wound healing time.2.Compared with the routine treatment of NPWT,NPWT combined with peracetic acid is more effective in the treatment of stage III/IV pressure injury,which provides a new method for the clinical treatment of chronic refractory wounds such as stress injury.It has higher clinical application value and social benefit.
