Objective: To observe the clinical efficacy of PLDD combined with spinal balance manipulation in the treatment of LDH,and to explore its mechanism and therapeutic advantages,so as to provide theoretical basis for the further clinical application and promotion of PLDD combined with spinal balance manipulation.Methods: This experiment adopts the principle of randomized controlled clinical research,will conform to the standard of 62 cases of LDH patients included in this study,according to random number table method,it can be divided into the treatment group and control group,two groups of patients,31 cases in treatment group given PLDD combined spinal adjustment scale technique,the control group given pure PLDD,both in the treatment of lumbar function of exercise routine guidance after 1 month,all of the patients before treatment,treatment after 1 month,3 months,6 months,1 year follow-up record patients' pain VAS score,JOA score,ODI score and lumbar curve segment activity index and operation,Serum inflammatory indexes CGRP,PGE2,NPY and SP were detected before and 1 month after treatment,JOA improvement rate was calculated 1 month after treatment,and efficacy was evaluated and Macnab was evaluated at 3 months,6 months and 1 year after treatment.Results: 1.There was no difference in general data between the two groups before treatment(P > 0.05).At 1 month after treatment,VAS pain score,JOA score,ODI score,lumbar spine curve index,operative segment activity and serum inflammatory factor score between the treatment group and the control group showed statistically significant differences(P < 0.05),and JOA score improvement rate showed statistically significant differences(P <0.05).2.Treatment after 3 months,6 months,1 year follow-up,the treatment group and control group patients pain VAS scores,JOA score,ODI score index and surgery,lumbar curve segment mobility comparative differences are statistically significant(P < 0.05),two groups of patients after 3 months,6 months,1 year,VAS pain score,JOA score,ODI score index and surgery,lumbar curve segment activity compared with preoperative and treatment in January,the differences were statistically significant(P < 0.05);6 months,1 year after treatment,two groups of patients with pain VAS score,JOA score and lumbar curve index and 3 months after surgery and treatment of segmental mobility in comparison,there were no statistically significant difference(P > 0.05),6 months,1 year after treatment,two groups of patients with ODI score compared with march after treatment,the difference statistically significant(P <0.05).3.The total good and good rates were 70.97%,77.42% and 83.87% in the treatment group at 3 months,6 months and 1 year after treatment,respectively,and were 54.84%,64.52% and 70.97% in the control group,respectively.There were statistically significant differences in the total good and good rates between the two groups at each time point of follow-up(P<0.05).Treatment after 3 months,6 months,1 year follow-up treatment group efficient Macnab score were 90.32%,93.55%,93.55%,cont rol group efficient Macnab score were 83.87%,87.09%,87.09%,two groups of efficient comparative difference was statistically significant(P < 0.05),and 1 year after treatment group patients with June Macnab score efficient comparison difference has no statistical significance(P > 0.05).Conclusion:PLDD combined with spine balance manipulation is superior to PLDD alone in the treatment of LDH,and the long-term effect is stable.PLDD combined with spine balance manipulation has the advantages of both treatment and treatment,and is an effective method for LDH,which is worthy of further application and promotion.
