Objective1.Introduce the English version of PURPOSE T pressure injury risk assessment tools, and thentranslate to Chinese and cultural adjustments, developing Chinese version of PURPOSE T,and then check the reliability and validity.2.To explore the ability of PURPOSE T to predict the occurrence of pressure injury in China, and to provide theoretical basis for the development of evaluation tools for pressure injury that are more suitable for clinical status in China.MethodsFirst, get in touch with the original evaluation form author, and obtain the author’s authorization to use the English version of PURPOSE T.Then,in accordance with the Brislin translation principle, the English version of PURPOSE T translation, back translation, cultural debugging, form the Chinese version of PURPOSE T;finally through the content effect The validity of PURPOSE T was analyzed by degree and validity. The reliability of the PURPOSE T evaluation tool was analyzed by Cronbach’s alpha,inter-rater reliability, and test-retest reliability.A random sampling method was used to select 200 critically ill patients admitted to a third-grade hospital in Zhenjiang from March 2018 to June 2018.The risk assessment of the patient’s pressure injury was assessed by two nurses using the PURPOSE T and Braden scales, and the patient’s basic condition was investigated. The patient’s skin condition and the results of the assessment of the assessment were registered. The sensitivity, specificity, positive likelihood ratio, and negative likelihood ratio were then used to compare the ability of the PURPOSE T and Braden scales to screen and identify the risk of pressure injury in critically ill patients.Data were entered and statistical analysis was performed using SPSS22.0statistical software.Results1.Chinese version of PURPOSE T reliability and validity verification results:Validity(1)Content validity Expert consultation shows that the content validity(S-CVI)of the Chinese version of the PURPOSE T total evaluation form is 0.969,and the content validity of each item(I-CVI)fluctuations in the range of 0.83~1.000;(2)criterion validity PURPOSE T and Braden scale correlation coefficient of mobility, activity, perception, nutritional status, moisture, etc.are 0.6,0.661,0.699,0.519,0.783;The correlation coefficient between PURPOSE T and Braden scale in predicting the risk of pressure injury was 0.407;the correlation coefficient between PURPOSE T and Waterlow scale in terms of nutritional status, low body mass index(BMI),high BMI and skin condition was 0.680 respectively 0.783,0.758,0.786.The correlation coefficient between the PURPOSET and Waterlow scales in predicting the risk of pressure injury was 0.577.Reliability:(1)Internal consistency reliability, the total Cronbach’s a coefficient of the evaluation table was 0.859.The Cronbach’s a coefficient for each item is 0.617 to 0.792.(2)The correlation coefficient between the assessors is 0.872.(3)The test reliability coefficient is 0.91.2.Chinese version of PURPOSE T predictive ability results in critically ill patients: The average time spent by the two groups of nurses using PURPOSE T was2.93 ± 3.14 points and 2.67 ± 3.47 points respectively;the two groups of nurses used Braden to estimate the time-consuming average The time was 3.26±2.27 points and3.43±2.65 points respectively. The use of PURPOSE T and Braden in nurses to evaluate the risk of pressure injury in critically ill patients was different, and the difference was statistically significant. The evaluation results of sensitivity, specificity, coincidence rate, positive predictive value, negative predictive value, positive likelihood ratio and negative likelihood ratio of PURPOSE T evaluation table are 84.6%,97.3%,96.5%,0.7,1.0,respectively.24.2,0.2;the sensitivity, specificity, coincidence rate, positive predictive value, negative predictive value, positive likelihood ratio, and negative likelihood ratio are 62.5%,47.4%,48%,0.0,1.0,respectively1.2,0.8.Studies in critically ill patients have shown that the coincidence rate of PURPOSE T with pressure injury is higher than that of Braden scale;the risk of pressure injury in risky patients measured by PURPOSE T score is higher than the Braden scale;PURPOSE T score The proportion of patients who are risk-free withoutpressure ulcers is higher than those measured negative for the Braden score.ConclusionThe Chinese version of PURPOSE T has proved to have good reliability and validity in clinical evaluation and evaluation in China. At the same time,PURPOSE T advocates paying attention to individual risk factors of patients, and more detailed and specific evaluation of patients with pressure injury risks is conducive to the development of individualized nursing programs..In the applied research of critically ill patients,it is shown that the Chinese version of PURPOSE T has a strong predictive ability in critically ill patients, and the sensitivity and specificity are balanced. This is a pressure to develop a clinical situation suitable for China. The damage risk assessment tool has a reference value.
