Objective: The standard procedure of cross-culture adaptation was used to develop the Falls Risk for Older People in the Community screening tool made by Australian. And researcher modified the items of the tool, to form Modified Falls Risk for Older People in the Community screening tool which was suitable for Chinese community elders. Then test its reliability and validity, aiming to provide a reference basis for the community medical health-care workers to screen falling dangerous persons and formulate preventions and interventions.Methods: 1.The linguistic translation of the Falls Risk for Older People in the Community screening tool was performed following the standard process of "Translation -Back translation-Cultural Adjustment-Preliminary Test-Field Test", and modified through Delphi method and Preliminary Test.2. Researcher and her companion conduct a survey on 211 community elders from 4 communities of Shanghai by sampling conveniently. The collected data were analyzed to reevaluate the reliability and validity of the Rating Scale. In the elderly population in Shanghai initial used the tool, according to the results of groups by the clinical gold standard diagnosis, drawn receiver operating characteristic curve, the largest Youden index corresponding to the sensitivity and specificity, made the best cut-off for highly falling, to identify its ability to predict fallers.3. Simultaneously, we used the data to analyse current situation and explore the main impacting factors of Chinese community elders. The data obtained from the above investigation were both inputted to SPSS17.0 for statistical treatment.Results: 1. The study first developed Falls Risk for Older People in the Community screening tool for Chinese which was composed of 19 assessment items. The internal consistency reliability was 0.804; the inter-rater reliability, content-related validity and discriminant validity were all good. Criterion-related validity of the Modified FROP-Com and Modified Falls Efficacy Scale was -0.824.2. According to the results of groups by the practical investigation, drawn receiver operating characteristic curve(ROC). When the cut-off value was 12, the largest Youden index was 0.592, and the sensitivity was 80.0%, the specificity was 79.2%, 80.7% positive predictive value and negative notice value of 78.4%.3. The results showed that the falling were not influenced by sex, age, BMI, marriage status, education level, house and living style, excluding use of mobility aids. Conclusion: On the basis of Delphi Method and preliminary test, the Modified Falls Risk for Older People in the Community screening tool was developed. It was confirmed scientific, reasonable and stable. The Modified FROP-Com is a simple, easy-to-use measurement. It has good scaling qualities. To identify the elderly prone to falls at an early stage, preferably before any falls have occurred, detailed evaluation of balance performance, activity level, and fear of falling need to be conducted. The Modified FROP-Com maybe an effective falls assessment tools for the elderly in China. It has laid a solid basis for further study. Further research is needed to establish the instruments'measurement properties.
