Therapeutic Effect Of Surgical Treatment And Hormone Therapy For Type?Autoimmune Pancreatitis

Objective: Type?autoimmune pancreatitis is more common in Asian populations.Imaging can be characterized by pancreatic swelling and focal masses.AIP,which is characterized by focal masses,is difficult to distinguish from primary pancreatic cancer.Sensitive to hormone therapy but with a high recurrence rate,the timing and method of surgical treatment remains controversial.The purpose of this study was to investigate the effects of hormone therapy and surgical treatment on the recurrence of type?AIP in different imaging findings and to analyze the clinical features and diagnosis and treatment experience of type?AIP.Method: The clinical data of 59 patients with type?AIP admitted to the General Surgery and Gastroenterology Department of Shengjing Hospital affiliated to China Medical University from January 2011 to February 2018 were collected and the prognosis was followed.According to the different pancreatic imaging findings and treatment methods of type 1 AIP patients The group was statistically analyzed for the recurrence rate after surgery and hormone therapy.Result: A total of 21 patients in the pancreatic swelling group,16 patients received hormone therapy,3 of them relapsed;5 patients underwent surgery for recurrence;Fisher's exact probability analysis found that hormone therapy and surgical treatment showed a pancreatic swelling of the whole pancreas There was no difference in the recurrence of type AIP(P=1).There were 38 cases of focal pancreatic mass,13 cases of hormone therapy,4 cases of recurrence;25 cases of surgery and 1 case of recurrence.Fisher's exact probability analysis showed that the recurrence rate of type?AIP was lower than that of hormone therapy(P=0.0382).Conclusion: 1.Surgical treatment can reduce the recurrence rate of type?AIP in imaging findings of focal pancreatic masses.2.Hormone therapy and surgical treatment have no difference in the recurrence of type?AIP with imaging findings of pancreatic swelling.If no hormonal treatment is contraindicated,hormone therapy can be used to treat such AIP.
