Objective: To analyze the clinical characteristics and treatment of bacterial liver abscess,and to improve the diagnosis and treatment of diseases.Methods: A retrospective analysis of 404 cases of hepatic abscess in our hospital from January 2012 to December 2018,general information,clinical symptoms,laboratory tests,pathogen examination,imaging examination and treatment characteristics.Results: General information: 404 cases of bacterial liver abscess,241 males,accounting for 59.7%,and 163 females,accounting for 40.3%.The age ranged from 17 to 89 years old with an average age of(59.6 ± 1.8)years.The high incidence of disease is 50 to 59 years old,a total of 120 cases,accounting for 29.7%.173 cases of diabetes,accounting for 59.5%.80 cases of hypertension,accounting for 27.5%.There were 67 cases of biliary tract disease,accounting for 16.6%.23 cases of malignant tumors,accounting for 7.9%.394 cases of fever,accounting for 97.5%,327 cases of chills and chills,accounting for 80.9%,256 cases of fatigue,accounting for 63.4%,203 cases of loss of appetite,accounting for 50.3%,194 cases of liver pain,accounting for 40%,nausea and vomiting 139 For example,34.4% and 31 cases of jaundice,accounting for7.7%.Pathogen distribution: A total of 231 blood cultures were cultured,with a positive rate of 28.1%.There were 165 cases of pus bacterial culture,and the positive rate was64.5%.There were 156 cases of bacterial culture positive and 122 strains of Klebsiella pneumoniae,accounting for 78.2%.15 strains of Escherichia coli,accounting for 9.6%.7 strains of Enterococcus faecium,accounting for 4.5%,6 strains of Acinetobacter baumannii,accounting for 3.8%;the main pathogen of bacterial liver abscess in our hospital in the past 7 years is Klebsiella pneumoniae,the proportion of Enterococcus and Enterobacter in recent two years Has risen.Klebsiella pneumoniae susceptibility:Klebsiella pneumoniae has been highly resistant to ampicillin for 7 years,and the resistance to cefuroxime,cefazolin,and cotrimoxazole is on the rise.Carbapenems(imipenem,meropenem),third-generation cephalosporins(ceftizoxone,cefotaxime,ceftazidime,etc.),quinolones(levofloxacin,ciprofloxacin,etc.)are highly sensitive Sex.Comparison of Klebsiella pneumoniae with or without diabetes: The diabetes group accounted for 49.2%,the males accounted for 75%,the females accounted for 25%,and the non-diabetic group accounted for 50.8%.51.6% of men and 48.4% of women.The incidence of diabetes in men was higher than that in non-diabetic group(P<0.05),while the non-diabetic group stayed longer(P=0.027);those with diabetes were more likely to have chills and chills(P=0.013),while those without diabetes were more likely to appear.Abdominal pain(P=0.023).The diabetic group was more likely to have basal fatty liver and tumor(P=0.015),(P=0.008).The incidence of hemoglobin and albumin in diabetic patients was higher than that in non-diabetic group(P<0.05),(P=0.022).Results:1.Between 2012 and 2018,404 bacterial liver abscesses occurred in men,with a good age range of 50 to 59 years.2.Klebsiella pneumoniae is the main pathogen and is resistant to carbapenems.3,the vast majority of antibacterial drugs are effective.
