Objective: The purpose of this study was to explore correlation between carotid atherosclerosis and other risk factors for hemorrhagic transformation after acute cerebral infarction,so as to clarify the mechanism of hemorrhage transformation,provide guidelines of treatment and improve the prognosis of patients with acute cerebral infarction.Methods: 263 patients with acute cerebral infarction who were hospitalized in Department of Neurology in the First Hospital of China Medical University from January2015 to June 2018 were successively recruited.According to the incidence,the patients were divided into 131 cases of cerebral infarction hemorrhage transformation(HT group)and 132 cases of non-hemorrhage transformation(NHT group).The differences of fasting blood glucose,triglyceride,low-density lipoprotein,total cholesterol,high-density lipoprotein,homocysteine,uric acid,blood pressure at admission,NHISS score,history of hypertension,history of diabetes,history of atrial fibrillation and carotid artery Crouse score between the two groups were analyzed.Univariate analysis was performed first,and multivariate logistic regression analysis was conducted for indicators with statistical significance,and P<0.05 was considered to be statistically significant.Results: 1.According to univariate analysis,NHISS score,carotid artery Crouse score,homocysteine level and history of atrial fibrillation in the HT group were significantly higher than those in the NHT group,with statistically significant differences(P<0.05).The levels of low-density lipoprotein and total cholesterol in the NHT group were significantly higher than those in the HT group,with statistically significant differences(P<0.05).However,there was no significant difference in gender,age,fasting blood glucose,uric acid,triglyceride,high-density lipoprotein,history of diabetes and history of hypertension between the two groups(P>0.05).2.Logistic multivariate regression analysis showed that history of atrial fibrillation,NHISS score and carotid atherosclerosis were independent risk factors for hemorrhage transformation of cerebral infarction(P<0.05).Conclusions: 1.NHISS score,history of atrial fibrillation,and carotid atherosclerosis inpatients with acute cerebral infarction were independent risk factors for HT.2.Carotid artery Crouse score in the HT group was significantly higher than that in the NHT group,presenting a positive correlation,and suggesting that carotid atherosclerosis can promote hemorrhage transformation.
