Objective:To determine the risk factors of carotid atherosclerosis in elderly patients with acute cerebral infarction based on single factor and multivariate analysis,provide scientific guidance for the development of clinical prevention of carotid atherosclerotic plaque formation.Methods:1.The research period from January 2016 to March 2019,the 60 patients with acute cerebral infarction without carotid atherosclerotic plaque were selected as the reference group,and 30 patients with carotid atherosclerosis were selected.Twenty elderly patients with acute cerebral infarction who had sclerotic unstable plaques were enrolled in the experimental group 1 and 30 patients with acute cerebral infarction with carotid atherosclerotic plaques were selected as the experimental group 2.2.Before the study was carried out,through the analysis of relevant research data,combined with its own clinical experience analysis,preliminary analysis of factors that may affect the formation of carotid sclerosing plaque in elderly patients with acute cerebral infarction,based on this determination to determine the need to include this study Relevant factors.After sorting and analysis,7 general data indicators and 12 test indicators were included.General data indicators: gender,age,smoking,drinking,hypertension,hyperlipidemia,diabetes;test indicators: fasting blood glucose,total cholesterol(TC),glycerol Triester(TG),high density lipoprotein(HDL-C),low density lipoprotein(LDL-C),homocysteine(Hcy),total bilirubin(TBIL),direct bilirubin(DBIL)Indirect bilirubin(IBIL),C-reactive protein(CRP),uric acid(UA)values,fibrinogen(Fib),and intima-media thickness(IMT)of the carotid artery.3.On this basis,firstly,single factor analysis was performed to compare the general data index and test index of the three groups of patients,and the single factor of carotid atherosclerosis in elderly patients with acute cerebral infarction was preliminarily determined.Then Logistic multivariate regression analysis was performed to determine the acute brain of the elderly and determined independent risk factors for carotid atherosclerosis in patients with infarction.Results:1.Univariate analysis:(1)In terms of general data comparison,there were no significant differences in gender,hyperlipidemia,coronary heart disease,and age index between the three groups(P>0.05).(2)In the comparison of hypertension,the reference group <experiment 2 group<experiment 1 group,the difference was statistically significant(P<0.05).(3)Regarding the test indicators,there were no significant differences in the fasting blood glucose,TC,TG,HDL-C,LDL-C,and DBIL indexes between the three groups(P>0.05).(4)In the comparison of TBIL and IBIL,the reference group> In the experimental group 2>Experiment 1 group,the difference was statistically significant(P<0.05).(5)In the comparison of Hcy,CRP,UA,Fib,the reference group <Experiment 2group <Experiment 1 group,the difference was statistically significant(P<0.05).2.IMT,the reference group <Experiment 2 group <Experiment 1 group,the difference was statistically significant(P<0.05).3.Multivariate regression analysis: Logistic multivariate regression analysis was performed on the statistically significant indicators of single factor comparison analysis.4. The results showed:(1)Age were not an independent risk factor for carotid atherosclerosis;(2)Smoking,drinking,hypertension,high Hcy,low TBIL,low IBIL,high CRP,high UA,high Fib were independent risk factors for carotid atherosclerosis.Smoking,drinking,hypertension,hcy,CRP,UA and Fib were positively correlated with carotid atherosclerosis.With the increase of Hcy,CRP,UA and Fib,it can promote the formation of carotid atherosclerotic plaque in patients with acute cerebral infarction,and the instability is gradually enhanced;(3)TBIL and IBIL are negatively correlated with carotid atherosclerosis and with the decrease of TBIL and IBIL,it can promote patients with acute cerebral infarction.Carotid atherosclerotic plaque is formed,and the instability is gradually enhanced.Conclusion:Smoking,drinking,hypertension,high Hcy,low TBIL,low IBIL,high CRP,high UA,high Fib are independent risk factors for carotid atherosclerosis in elderly patients with acute cerebral infarction,which will increase carotid atherosclerosis and the risk of formation of hardened plaque.In the clinical,special attention should be paid to the above risk factors,and targeted prevention and management should be carried out to prevent the formation of carotid plaques.
