PurposeA retrospective clinical analysis of 62 patients with ? and ? lumbar spondylolisthesis with mistlif(minimally invasive transforaminal lumbar interbody fusion)was performed to analyze the reset effect and influencing factors of mistlif in the treatment of ? and ? lumbar spondylolisthesis.Materials and MethodsThe subjects were 62 cases of ? and ? lumbar spondylolisthesis admitted to our hospital from September 2015 to September 2018,including 22 males and 40 females,aged 38-80 years,with an average of 59.96±10.19 years old.All were L4 or L5 single-segment mild lumbar spondylolisthesis,including 32 cases of L4 slippage,30 cases of L5 vertebral spondylolisthesis,38 cases of I degree,24 cases of II degree,26 cases of isthmic split type and 36 cases of degenerative type.Subsequently,the preoperative and postoperative spondylolisthesis,preoperative spondylolisthesis,preoperative detachment angle(positive forward angle was negative,negative backward angle was negative),sagittal shape were measured with preoperative and postoperative lumbar lateral radiographs.The angle of the vertebral pedicle screw,the angle of the vertebral pedicle screw in the sagittal plane,and the angle of the bilateral longitudinal rod.The preoperative lumbar CT scan was used to determine whether the lumbar spinal canal was absolutely stenotic(the anteroposterior diameter of the spinal canal was less than 13 mm).At the same time,the age,gender,slippage segment(L4 or L5),and the cause of slippage(both isthmus fissure or degeneration)were recorded.The statistical method uses spss22.0 software,the counting data is expressed by the frequency,the measurement data is expressed by the mean±standard deviation,the grouping measurement resets the satisfaction group and the resetting rate of the unsatisfactory group,and evaluates the reset effect.The statistically significant influencing factors were included in the multivariate logstic regression,and the relevant factors of the reduction effect were evaluated(P<0.05 was considered statistically significant).ResultThe reset rate of the satisfactory group was 75.55%,and the reset rate of the unsatisfactory group was 45.98%.Univariate logistic regression analysis of gender,preoperative detachment angle,spondylolisthesis,presence or absence of lumbar spinal stenosis,sagittal detachment vertebral pedicle screw angle,sagittal subvertebral pedicle screw clamp There was no significant difference in the comparison of angle and other factors(P>0.05).There were significant differences in age,the cause of slippage,and the angle between the bilateral longitudinal rods(P<0.05).Multivariate logistic regression analysis of the above-mentioned related factors found that age and slippage were the main risk factors affecting postoperative reduction.in conclusionBy analyzing the case data of 62 patients with ? and ? degree lumbar spondylolisthesis treated with mistlif,the preoperative and postoperative spondylolisthesis rate was calculated,and the selected influencing factors were included in the logistic regression equation.The conclusion was drawn: preoperative detachment angle and influencing factors Infusion of mistilf for the treatment of ? and ? degree lumbar spondylolisthesis,the immediate reduction effect after surgery is significant.The older age and isthmus is the main risk factor,which has a negative impact on the satisfaction of the reduction.These factors should be paid attention to during surgery.
