Objective:In this study,the changes of TCD,ICP and Rotterdam CT score in patients with STBI for 7 days after surgery were continuously and dynamically monitored,so as to investigate the clinical value and significance of TCD combined with ICP as well as Rotterdam CT score in the evaluation of patients' condition and prognosis after STBI.Methods:25 patients with STBI who met inclusion and exclusion criteria were involved in by designing self-controlled study.Record the Rotterdam CT score of the patients with the most severe reaction within 72 hours after admission,and continuously monitor the patients' TCD and ICP for 7 days after surgery.At 3 months after injury,the patients were divided into four groups,Good Prognosis Group,Moderate Disability Group,Severe Disability Group and Death Group,according to the GOS by telephone or outpatient follow-up.Results:ICP and the Pulsatility Index(PI)were negatively correlated with the prognosis of patients(P<0.05): for patients in Good Prognosis Group and Moderate Disability Group,the PI and ICP increased continuously with time(P<0.05),and the PI was positively correlated with ICP(P<0.05).Rotterdam CT score was negatively correlated with the prognosis of patients(P<0.05);and Rotterdam CT score was positively correlated with ICP(P<0.05),but there was no correlation between Rotterdam CT score and PI(P>0.05).Conclusion:the pathophysiological process of severe TBI is complex,with the continuous occurrence of complications in the treatment process,the patient's condition also gradually aggravated,affecting the prognosis of the patient.Therefore,ICP monitoring combined with TCD as well as Rotterdam CT score can evaluate the severity and predict the prognosis of the patients with STBI from multiple levels and perspectives,and provide individualized and precise clinical intervention measures for the treatment of patients with STBI.
