Objective: Traumatic Brain Injury(TBI)is the damage of normal brain function caused by direct or indirect effects of external forces such as impact,strike or penetrating head injury.With the development of modern society,severe trauma has gradually developed into a major issue in the field of global public health.It will directly affect the recovery and prognosis of patients with severe traumatic brain injury after operation if the intracranial hypertension can not be detected and handled in time.Invasive intracranial pressure monitoring technology can monitor the continuous dynamic intracranial pressure(ICP)of patients with brain injury after operation.Through the monitoring data,clinical neurosurgeons can timely and accurately grasp the progress of patients' condition,so as to adjust the clinical treatment plan as early as possible and eventually improve the overall therapeutic effect of patients.We conducted a comparative study for more than three years to observe the effect and explore the clinical application value of invasive dynamic intracranial pressure monitoring technology in the treatment of patients with severe traumatic brain injury after operation,and to evaluate the prognosis of patients.Methods: From January 2015 to April 2018,64 patients with severe traumatic brain injury were randomly divided into control group and observation group.The selected patients were all patients with severe craniocerebral injury who underwent craniotomy.Clinical datas included gender,age,type of injury,GCS score at admission,main clinical symptoms and positive signs of nervous system,past medical history,imaging findings,important laboratory indicators,types of complications and GOS score at half a year after operation.The patients in the observation group were monitored by invasive dynamic intracranial pressure monitoring.The monitoring equipment was Codman?intracranial pressure monitor produced by Johnson & Johnson Company of the United States.The diameter of pupils and vital signs were closely observed after operation.Changes of intracranial pressure were continuously monitored and recorded,and the treatment plan was adjusted in time according to the above changes.Then the control group was not monitored by invasive dynamic intracranial pressure monitoring technology.Clinicians made empirical treatment by observing the clinical symptoms,signs and imaging findings of patients.The clinical effects and complications of the two groups were compared and analyzed.Results: The clinical effect of the observation group was significantly better than that of the control group,P < 0.05.The number of cases of renal function injury,the utility timeof mannitol and the daily dosage of mannitol in the observation group were significantly less than those in the control group,P < 0.05.And the incidence of complications in the observation group(12.50%)was significantly lower than that in the control group(34.38),and P < 0.05Conclusion: The change of the ICP in patients with severe craniocerebral injury after operation can be easlily grasped through the dynamic invasive intracranial pressure monitoring technology.According to the results of monitoring data,targeted intracranial pressure control therapy can be carried out to effectively control the patient's condition.On the basis of improving the curative effect of patients,the incidence of various complications is reduced,so as to effectively improve the prognosis of patients.
