Psoriasis is a common chronic relapsing inflammatory skin disease.The lesions of psoriasis often happen on the certain parts of the body such as the scalp,the trunk and the stretch of limbs.The mechanism of this phenomenon is unclear.However,these parts of our body have the similar characteristic that the tension on these parts of our body is higher than others.In recent years,the study found that the skin barrier function is the main factor that affects the occurrence and development of psoriasis.Additionally,there are a variety of mechanisms for participating in this process.Then repairing the skin barrier function helps to alleviate the lesion and delay the recurrence.The mechanical tension on the skin surface may act directly on the physical and/or biological changes of the skin,therefore we speculate that the mechanism why psoriasis lesions often occurs in the scalp and limbs stretch side is the impairing skin barrier function damaged by the surface mechanical tension.Objective: To explore whether the skin tension on the body surface can promote psoriasis by affecting skin barrier function.Methods: The skin barrier function of patients with psoriasis and dilator was examined to observe the function of skin barrier.The submicroscopic structure of epidermal skin was observed to make sure whether the skin barrier function of patients with psoriasis and dilator was impaired.At the same time,the psoriasis mice model and the Imiquimod-induced Psoriasis-like Mouse Models,which were copied with the dilator inside,were established to observe the phenotype,barrier function and the changes of related molecules.Results: The result of skin barrier function measuring to psoriasis skin lesions and dilatation site is that the skin barrier function is impaired.It shows that the TEWL increased,while moisture and lipid decreased.The transmission electron microscope observation of psoriasis lesions and skin expander lesions shows that the skin barrier function related structures have changed,for example: the lamellar body is irregular,deformity,and swelling,membranous structure inside the lamellar body is disorder,electron density of the lamellar body increases,the space channel closure in corneodesmosomes stops the intercellular lipid transportation at stratum corneum.It is also observed in dilator mice mode that the skin barrier function is impaired in the expander lesions and the lamellar body is abnormal.When the Imiquimod-induced Psoriasis-like Mouse Models were established,the expression of some protein gene related to epidermal barrier is inhibited.It is obvious that the expander skin in mice is suffering from psoriasis easier.Conclusions: Through clinical skin barrier function test,submicrostructure observation,and dilator animal model studies,this research found that the phenomenon of higher tension on the surface of the skin is more likely to suffer from psoriasis is related to its skin barrier function damage,and surface tension of the skin promotes the development of psoriasis by influencing the skin barrier function.
