Background and objective: At present,the consensus in the international medical community is that the mortality rate of gastric cancer is high,and it has become one of the most important cancer-related death factors,ranking third,and most importantly,as the fifth most common malignant tumor in the world,most of them.The patient has had a distant metastasis at the time of the first clear pathological diagnosis.For this part of the patient,the most important treatment is systemic chemotherapy.Although it cannot be completely cured,it stabilizes the disease,delays the progress,and improves the quality of life.Prolonging survival is the most meaningful treatment.Therefore,a highly specific biomarker that indicates prognosis is important for these patients.Thioredoxin Reductase(Trx R / TR)itself has many biological activities in tumorigenesis,development,and spread.The different stages of the transfer play a very important role.It can maintain the balance of redox reaction inside the cell,reverse the damage caused by various types of oxidative stress,accelerate the cell division and growth,increase the ability of tumor cells to resist apoptosis,and accelerate the metastasis of cancer cells.Diffusion is a very desirable potential biomarker.However,the specific relationship between the level of TR and the short-term efficacy of anti-tumor therapy and the prognosis of cancer patients is not yet clear.The purpose of this study is to explore the relationship between TR and the short-term efficacy and prognosis of patients with advanced gastric cancer,and to assess the level of TR.The possibility of being a new prognostic indicator.Methods: This study was conducted with cancer patients from Jiangsu Cancer Hospital.The enrollment time was from November to November 2018,2017,for a total of one year.The subjects were eligible for advanced gastric cancer patients.All patients enrolled in the study should have at least 2 courses of anti-tumor treatment on schedule,and the therapeutic efficacy should be evaluated after 2 courses of treatment.The evaluation criteria are Response Evaluation Criteria in Solid Tumors(RECIST).TR levels and carcinoembryonic antigen(CEA)levels were measured before the first anti-tumor treatment,and TR levels and CEA levels were measured again in the first evaluation.All patients were followed up from the enrollment,and the survival status and loss of follow-up of all follow-up patients were recorded.All patients' follow-up information was analyzed and processed using Statistical Software for Social Sciences(SPSS)24.0.Results:A total of 202 patients were enrolled in the study,including 57 females and 155 males,the youngest being 30 years old and the oldest being 81 years old.The patients were divided into two groups according to the pre-treatment TR level: TR? before treatment There were no significant differences in gender,age,metastatic location,pathological type,and differentiation between the 8 U/m L group and the pre-treatment TR>8 U/m L group(P>0.05).In the first evaluation of efficacy,37 patients were evaluated for PR,105 patients with SD were evaluated for efficacy,and 142 patients with DCR were evaluated.The mean TR decreased after treatment.The difference was not statistically significant.(P=0.098),the mean CEA decreased after treatment,the difference was statistically significant(P=0.000);60 patients with PD evaluated the effect,the mean TR increased after treatment,the difference was statistically significant(P=0.031).The mean CEA increased after treatment,and the difference was statistically significant(P=0.000).All the 202 patients enrolled in the study were followed up.The last follow-up time was March 2019.96 patients were still alive,96 patients died,and 10 patients were lost to follow-up.The survival status and survival time of all patients were counted.The Kaplan-Meier method of SPSS software statistically analyzed the relationship between the influencing factors of all patients and the survival of advanced gastric cancer,and performed a survival curve.The results showed: age,gender,pathological type,degree of differentiation,metastatic site,There was no significant difference in pre-treatment TR levels,TR trends,treatment effects,pre-treatment CEA levels,CEA trends,and median survival(P>0.05).COX regression analysis was performed using SPSS software to analyze the relationship between the influencing factors of all patients and the survival of advanced gastric cancer.The results showed: age,gender,pathological type,degree of differentiation,metastatic site,pre-treatment TR level,TR change trend,There was no independent correlation between CEA trends and prognosis,and pre-treatment CEA levels were independent factors influencing the prognosis of patients with advanced gastric cancer.Conclusion: The results show that TR levels can predict the short-term efficacy of patients with advanced gastric cancer,and CEA can predict the short-term efficacy and prognosis of patients with advanced gastric cancer.
