Objective:To review the long-term oncologic outcomes and short-termefficiency of laparoscopy-assisted gastrectomy(LAG) for advanced gastriccancer, indicating the clinical value and indication of this method ofoperation, summarizing the clinical experience of it, finally improving thedevelopment and promotion of laparoscopic surgical techniques.Thus, we canreduce the complications of operation and improve the quality of life andsurvival rates of patients.Methods: The authors retrospectively analyzed108patients whounderwent LAG for advanced gastric cancer from March2008to November2011in our institution. We reviewed clinicopathological features, postoperativeoutcomes, mortality and morbidity, recurrence and survival of LAG based onAJCC (American Joint Committee on Cancer)2010tumor staging system foradvanced gastric cancer.Results,:The last time of follow-up is January2014,108operations wereperformed,98patients were followed-up, the follow-up rate was90.7%(98/108), median follow-up period was30months (range6~70months).26patients died and3patients developed recurrence during the follow-up period.The overall recurrence rate was26.9(29/108), in which the recurrence ratewas0%(0/6) of IB,18.8%(6/32) of IIA,30%(6/20) of IIB,30.8%(8/26) ofIIIA,37.5%(9/24)of IIIB,respectively. The overall1,2,3-year survival rateswere95.4%,88.9%,73.8%.,and1,2,3-year progression-free survival rateswere93.5%,83.3%,73.8%,respectively.Conclusion: LAG is a safe and feasible surgical procedure combined with minimal trauma、 fast recovery and fewer postoperative complications inshort-term. The survival rate of our series is higher,and the long-termoutcomes is reliable.The age and tumor stage are independent risk factors forlong-term oncologic outcomes, and there are significant differences insurvival rate among different stages. In this study, patients in stage IIIdeveloped recurrence earlier, and lead a short time after operation. These maybe associated with the late clinical staging.
