Objective: Analysis of related cases of advanced gastric cancer treated by laparoscopic or open radical D2 radical gastrectomy. Review of relevant index study of intraoperative and postoperative. To review the clinical value of laparoscopy-assisted gastrectomy(LAG) for advanced gastric cancer.Methods: Select the patients with gastric cancer surgery in the first affiliated hospital of Wan Nan Medical College from January 2014 to December 2014. Including 32 cases of laparoscopic D2 radical gastrectomy(LG group),48 cases of open D2 radical gastrectomy(OG group).Analysis the efficacy and compare the mean operation time, blood loss, time for passage of flatus, bed time after operation,liquid food intake, peritoneal cavity drainage tube extraction, postoperative hospital stay, the number of lymph nodes dissected, postoperative complications between two groups of patients.Results: Operation time of LG group is(210.94±17.62)min, two cases of postoperative complications, Operation time of OG group is(204.17±21.30)min,five cases of postoperative complications. Operation time and postoperative complications had no significant difference between LG group and OG group(P>0.05). Blood loss,time for passage of flatus, bed time after operation,liquid food intake, peritoneal cavity drainagetube extraction, postoperative hospital stay had significant difference between LG group and OG group(P<0.05).The number of lymph nodes dissected of LG group is(24.22±5.11), OG group is(25.19±8.93), they had no significant difference between the two groups(P>0.05).Conclusion: Laparoscopic assisted D2 radical gastrectomy for locally advanced gastric cancer has a similar short-term effectiveness as open D2 radical gastrectomy. Laparoscopic assisted D2 radical gastrectomy has the advantages of minimal invasion, rapid recovery. Long-term efficacy need Large randomized controlled trials and long time to be clear.
