Clinical Study Of Modified Huo Luo Xiao Ling Dan Decoction On The Blood Hypercoagulability After Total Hip Arthroplasty In Patients

ObjectiveTo explore the clinical effect of modified huo luo xiao ling dan decoction effect on postoperative blood hypercoagulability in patients undergoing total hip arthroplasty and to evaluate its safety.And provides clinical basis for the prevention of deep veno us thrombosisin lower extremities by traditional Chinese medicine.MethodsA total of 60 patients who received total hip arthroplasty from May 2016 to Dec ember 2018 in department 1 of bone injury of People's Hospital Affiliated to Fujian University of Traditional Chinese Medicine were collected,they were divided into two groups of control group and test group as per the random digital table method.The operation was performed by the same group of clinicians and epidural anesth esia was used in both groups.The control group was treated with hypodermic injection of low molecular weight heparin sodium and physical therapy,until the 14 th day after operation.On the basis of routine treatment in the control group,the experimental gro up was treated with modified Huo luo xiao ling dan decoction on the second day afte r operation,until the 14 th day after operation.Four items of coagulation,D-dimer level,limb circumference difference and VAS pain score were measured on the 2nd and 14 th day after operation in the two groups.And the four items of coagulation and the leve l of D-dimer were measured.On the 7th day after operation,the blood flow velocity of femoral vein was calculated by Doppler ultrasonography of lower extremities to evalu ate the state of blood hypercoagulability and incidence of deep venous thrombosis in l ower extremities.To observe the amount of bleeding,drainage fluid and incision after o peration were measured.The index of bleeding volume was used to evaluate its safety.All data were analyzed by SPSS22.0 software.Research result1.the levels of APTT,PT,TT in the two groups were significantly prolongedthe lev els of Fib,D-dimer in the two groups were significantly lower than those in the contr ol group(P<0.05),and the improvement effect of the experimental group was better th an that of the control group(P<0.05).2.both groups could significantly improve the degree of limb swelling after operat ion(P<0.05)and reduce postoperative pain(P<0.05).The difference was statistically si gnificant.The experimental group was superior to the control group.3.The experimental group could accelerate the velocity of femoral vein(P>0.05).T here was no significant difference in bleeding volume between the two groups(P>0.05),and there was no significant difference in the incidence of DVT between the two g roups(P>0.05).ConclusionThe combination of modified Huo luo xiao ling dan decocotion and low-molecula r-weight heparin sodium could significantly reduce the levels of D-dimer and Fib,prolo ng the time of APTT,PT,TT,Acceleration of femoral vein velocity,and improve the blo od hypercoagulability after total hip arthroplasty surgery.The modified Huo luo xiao li ng dan decocotion can reduce the pain caused by the postoperative wound of the pati ent,improve the swelling degree of the limb after the operation,without aggravating thedisadvantages of postoperative bleeding,and it is safe and reliable,and is worthy ofclini cal popularization and use.
