Study On Anxiety Disorder And Its Related Factors In Patients With Chronic Heart Failure

ObjectiveTo investigate the occurrence of anxiety disorder and related factors in CHF patients,and to find out and timely intervention,to judge the development trend of the disease,provide clinical guidance for the treatment plan,To provide further scientific evidence for improving the prognosis of CHF patients,reducing the risk of long-term death and clinical research.Methods(1)Subjects:A cross-sectional study was conducted to collect 284 patients with CHF who were hospitalized in the Department of Cardiology of Shaanxi Provincial People's Hospital from November 2017 to December 2018;(2)Data collection:Collect clinical baseline data of all included population,including gender,age,height,weight,BMI,education level,town type,blood routine,blood lipids,liver and kidney function,thyroid function,troponin I(TnIDx),BNP,HbA1c,LVEF and other indicators.(3)Anxiety assessment and grouping:HAMA was used to assess the anxiety of patients,and those with HAMA score of>14 were defined as having anxiety disorders.(4)Statistical methods:All data were statistically processed using SPSS 18.0 statistical software.The measurement data that conforms to the normal distribution is expressed as mean±standard deviation(x±s),and the count data is expressed as a percentage.CHF patients were divided into anxiety group and non-anxiety group according to anxiety score.The t test was used for the measurement data and the?~2 test was used for the measurement data.The Pearson correlation analysis was used between the anxiety score and the measurement data that conformed to the normal distribution.Spearman correlation analysis was used between the measured data of normal distribution;Logistic regression analysis was used between anxiety and the included indicators to study the influencing factors of anxiety disorder in CHF.Results(1)Among 284 patients with CHF,there were 100 patients in the anxiety group,and the prevalence of anxiety was 35.2%.Among them,174 were males,accounting for 61.3%,the prevalence of anxiety was 28.2%,110 were females,accounting for 38.7%,and the prevalence rate was 46.4%.(2)Univariate analysis showed that there was a statistically significant difference in gender,NEU,HB,BUN,CRE,Cys C,TBIL,DBIL,ALB,cardiac function grading,LVEF,and BNP in the anxiety group compared with the non-anxiety group(P<0.05).ANOVA analysis of variance showed that there were significant differences in BNP(F=17.819,P<0.001),anxiety score(F=11.429,P<0.001),and ALB(F=3.252,P=0.040)in patients with different cardiac function grading.significance.(3)Correlation analysis showed that the anxiety score was positively correlated with BNP,NEU,Cys C,DBIL and TBIL of CHF;it was negatively correlated with RBC,HB,TP,ALB and LVEF.(4)Logistic regression analysis showed that BNP(OR=1.001,P<0.001),gender(OR=3.634,P<0.001),BUN(OR=1.190,P=0.011),cardiac function grading(OR=2.336,P=0.001),DBIL(OR=1.056,P=0.021)were significantly associated with anxiety disorder in CHF,but there was no significant correlation between Cys C,HB,TP,ALB,NEU and anxiety(P>0.05).ConclusionThe prevalence of anxiety in CHF patients is higher;female CHF patients are more likely to be associated with anxiety than male CHF patients;gender,cardiac function grading,BNP,BUN,DBIL and CHF are associated with anxiety,and CHF is significantly corrected after adjusting for relevant factors.The incidence of anxiety in patients is greater.
