| Objective:1.Investigation of chronic heart failure (CHF) patients with anxiety and depressioncondition.2.Yixin Decoction on patients with anxiety and quality of life in CHF, to seek theeffective method for treatment of CHF with traditional Chinese medicine combinedwith anxiety and depression.Methods:The study is mainly divided into two parts:1.In155cases of chronic heart failure patients with Hamilton Anxiety Scale (HAMA)and Hamilton Depression Scale (HAMD)for anxiety and depression score, than make acomparative analysis;2.Belonging to liver ai stagnation and spleen deficiency blood stasis of the chronic heartfailure complicated with anxiety and depression and TCM syndrome in60patients wererandomly divided into study group and control group with30cases in each group, thecontrol group were given western medicine routine anti heart failure treatment, thestudy group on the basis of routine anti heart failure therapy plus Yixin Decoction, onedose daily, morning and evening two pm blunt. The two groups were treated for4weeks. Before and after treatment do TCM syndrome score, anxiety, depression andquality of life integral comparison; cardiac function (NYHA), echocardiographycomparison; serum BNP concentration detection comparison.3.All material using SPSS17.0statistical software for statistical data processinganalysis. Measurement data said to the t test for significance test, counting data by χ2test, the Ridit analysis level data, when P<0.05, has a significant difference in statistics;When P<0.01, has the extremely significant difference in statistics.Results: 1.In155cases of heart failure of the patients surveyed,34cases complicated withanxiety, accounted for21.9﹪;47cases with depression, accounting for30.0﹪;21patients with depression and anxiety coexist, accounted for13.5﹪. Anxiety scorecomparison, female is significantly higher than male, single is significantly higher thanmarried, hypertension diabetes significantly higher hypertension diabetes, significantlyhigher than the junior high school the following education level of junior high schoolabove culture, the difference was statistically significant(P<0.01or P <0.05); Depressionscore comparison, women are significantly high than men, is greater than or equal to65-year-old was obviously higher than less than65-year-old, married the significantlyhigher than the single, significantly higher than the high blood pressure, diabetes is notthe combining high blood pressure, diabetes, patients with higher NYHA heart functionclassification, the higher the incidence of depression,the difference was statisticallysignificant(P<0.01or P<0.05).2.①The treatment group before and after treatment comparison: After the treatment ofanxiety and depression scores and quality of life scores were reduced; clinicalsymptoms and signs, LVEF echocardiographic indicators have improved; serum BNPlevels were significantly decreased compared with before treatment difference wasstatistically significant (P<0.01).②The control group before and after treatment comparison: After the treatment ofanxiety depression scores than before treatment had no significant decline comparedwith before treatment difference was not statistically significant (P>0.05); Aftertreatment of TCM symptoms and signs, echocardiography LVEF index were improved,the quality of life score decreased significantly, serum BNP level dropped significantly,compared with before treatment differences significant statistical significance (P<0.01).③After treatment the treatment group compared with control group: the curative effectof TCM symptoms and signs improved after treatment, the NYHA heart function,ultrasonic cardiogram LVEF indexes improved, serum BNP levels drop, treatment groupcurative effect is better than that of control group, the difference statistically significant (P<0.05); After treatment of TCM syndrome integral, anxiety, depression, quality oflife, integral reduced, the treatment group decreased more significantly, with significantdifference statistically significant (P<0.01); No serious adverse reaction occurred inboth groups.Conclusion:1.In patients with chronic heart failure in the hospital, a high incidence of anxietydepression; Women, low degree of culture, single, hypertension, diabetes mellituspatients more occur anxiety disorders. Female, age, single, hypertension, diabetesmellitus, poor heart function of patients are more likely to occur depressive disorder.2.Yixin Decoction granule combined with normal heart failure therapy can improve theclinical curative effect of CHF with anxiety depression disorder, effectively alleviate theanxiety of patients with depression, improuve the quality of life of the patients. |
