Objective:To investigate the risk factors associated with cognitive dysfunction in patients with type 2 diabetes mellitus?T2DM?and to provide a scientific strategy for prevention and treatment of cognitive dysfunction in patients with T2DM.Methods:Sixty-five patients with T2DM were enrolled according to the inclusion exclusion criteria.After informed consent,the scores were assessed using the Simple Intelligence Mental Status Scale?MMSE?.The subjects were divided into two groups according to the scoring criteria:?27 was divided into normal cognitive function group?27cases?,and<27 was classified into cognitive dysfunction group?38 cases?.Collect general information such as age,gender,body mass index?BMI?,and smoking;collect glycosylated hemoglobin?HbA1C?,fasting blood glucose?FPG?,fasting C-peptide,fasting insulin,2 hours postprandial insulin,2 hours postprandial C-peptide 2 hours postprandial blood glucose,TG,free triiodothyronine?FT3?,free thyroxine?FT4?,thyroid stimulating hormone?TSH?,cholesterol?CHOL?,low density lipoprotein?LDL-C?,high Biochemical indicators such as density lipoprotein HDL-C,uric acid?UA?,serum creatinine?CRE?;ask patients whether the treatment plan is only oral metformin.Finally,statistical analysis was performed to analyze the statistically significant factors in the univariate analysis into the multivariate logistic analysis model,and then calculate the OR value and 95%CI to find out the independent risk factors affecting the cognitive function of the two groups and the degree of their influence.Results:1.According to statistical analysis,the differences in general demographic data such as age,gender,BMI,and smoking status between the cognitive dysfunction group and the normal cognitive function group in this study were not statistically significant;2.FPG,2 hours postprandial insulin,2 hours postprandial C peptide,2 hours postprandial blood glucose,HDL-C,CRE,FT3,FT4 and TSH differed between cognitive dysfunction group and normal cognitive function group.Statistical significance;3.Univariate analysis revealed hypertension(?2=5.54,P=0.019),T2DM duration(?2=10.67,P=0.001),HbA1C level?t=-2.11,P=0.035?,insulin resistance index?t=-2.06,P=0.039?,triglycerides?t=-1.98,P=0.047?,cholesterol levels?t=-0.23,P=0.042?,etc.,were statistically different between the two groups,and then included in multivariate logistic regression.Analyze the model to find out the influencing factors of cognitive dysfunction in patients with T2DM.Whether they have hypertension OR 95%CI[6.28?1.5725.19?],T2DM time OR 95%CI[12.36?2.4163.52?]is T2DM patients Independent risk factors for cognitive dysfunction;4.UA levels were statistically different between cognitive dysfunction group and normal cognitive function group[?351.07±76.42?vs.?311.40±73.11,P=0.038?].Multivariate logistic regression analysis found that it may be the protective factor of cognitive dysfunction in patients with T2DM was OR?95%CI?0.89?0.880.99?;5.There was a statistically significant difference in oral drug metformin between the cognitive dysfunction group and the normal cognitive function group?P<0.05?.Conclusion:1.T2DM with hypertension and longer duration of diabetes is an independent risk factor for T2DM with cognitive dysfunction;poor glycemic control,hyperlipidemia,and insulin resistance are risk factors for T2DM cognitive impairment,indicating control of blood pressure and blood glucose.Maintaining normal levels of blood lipids helps prevent T2DM cognitive dysfunction;2.Uric acid may be a protective factor of cognitive function in patients with T2DM;metformin may have protective effect on cognitive function in patients with T2DM.
