Observation On The Curative Effect Of Wuling Capsule In The Treatment Of Chronic Tinnitus With Emotional Disorder

Objective:1.To observe the efficacy and safety of Wuling capsule in the treatment of chronic tinnitus with emotional disorder;2.To assess the relationship between the severity of chronic tinnitus and emotional disorders and sleep;3.To evaluate the improvement of Wuling capsule on tinnitus symptoms,depression,anxiety and poor sleep.Methods:From December 2017 to March 2019,80 patients with chronic tinnitus with emotional disorder were treated in the out-patient department of otolaryngology,affiliated hospital of Yan'an University,They were randomly divided into experimental group and control group.There were 23 males and 17 females in the experimental group,The mean age was45.15±13.040years old and the course of disease was 13.95±12.476 months,There were12 cases in the left ear,14 cases in the right ear and 14 cases in both ears;In the control group,26 males and 14 females,average age44.63±15.777 years,course of disease 11.58±8.973 months,left ear 14 cases,right ear 15 cases,double ear 11 cases.The experimental group was given Wuling capsule(0.99g/times,3 times a day)and Ginadol tablets(40mg/times,3 times per day),the control group was given glutamine tablets(20 mg/times,3times a day)and Ginadol tablets(40mg/times,3 times a day),THI?HAMA?HAMD?PSQI scores were completed before treatment and 2 weeks,4 weeks and 8 weeks after treatment.Results:1.Comparison of general data:there was no significant difference in the baseline data of age,sex,course of disease,ear differentiation between the experimental group and the control group(P>0.05).Before the treatment,the data of THI?HAMA?HAMD?PSQI between the two groups were not statistically significant,the data of the two groups were balanced and comparable.2.The rank correlation analysis was carried out between the THI score level and the corresponding HAMA?HAMD?PSQI score,The spearman correlation coefficients were0.844,0.844 and 0.853 respectively,P<0.05,indicating that the HAMA?HAMD?PSQI score had a significant positive correlation with the THI grade.3.The scores of THI grade and corresponding HAMA?HAMD?PSQI scale were tested by kruskal-wallis H rank test,and the scores of?~2 were 56.266,57.670,57.455,respectively,which indicated that The scores of tinnitus severity were different,and the related emotional disorders and sleep were different.4.The THI?HAMA?HAMD?PSQI values between experimental group and control group showed a downward trend,and the processing factors(that is the two groups selected different drugs)were analyzed,P<0.05,,It can be concluded that the choice of different drugs has different effect on the decrease of the scores of each scale.The time factors were analyzed statistically,P<0.05,It can be concluded that the scores of the two groups were not all equal at different time before and after treatment.By statistical analysis of the interaction effect of time and group,P<0.05,it can be concluded that the scale scores of the two groups are different at different time points.The scores of the two groups were further compared at 2 weeks,4 weeks and 8 weeks after treatment,At the end of the second week of treatment,the THI?HAMA?HAMD scores of the two groups were compared,P>0.05,PSQI score comparison,P<0.05,The data of the two groups were compared at 6weeks and 8 weeks,the P value was less than 0.05,the difference was statistically significant.The effective rate of the experimental group was higher than that of the control group,the difference was statistically significant(P<0.05).5.There were 2 cases with adverse reactions in the experimental group(5.00%),and 1case in the control group(2.50%).There was no significant difference between the two groups(P>0.05).Conclusion:1.Anxiety,depression and sleep disorders are associated with the severity of chronic tinnitus.the higher the level of tinnitus,the more severe the mood disorder and the worse the sleep quality.2.Wuling capsule can improve the severity of chronic tinnitus with emotional disorders and related emotional disorders,and improve the quality of sleep,reduce the THI?HAMA?HAMD?PSQI scale score,the clinical efficacy is better than the control group,with the prolongation of treatment time,the curative effect is more significant.The incidence of adverse reactions was lower.
