Objective: To investigate the characteristics of emotional central nervous system in patients with idiopathic tinnitus and the correlation with the severity of tinnitus.Methods: 24 cases of idiopathic tinnitus and 22 cases of control group were compared,tinnitus patients were tinnitus matching frequency test,tinnitus disability assessment scale(THI),anxiety self-rating scale(SAS)and depression self-rating scale(SDS)for the severity of tinnitus score.The stimulation frequency was 120 d B,and the stimulation frequency of tinnitus group was the matching frequency of tinnitus patients.The stimulation frequency of the control group was 1KHz.The orbital frontal cortex,amygdala and anterior cingulate gyrus were made respectively.The data were processed by spm8 software to obtain the activation volume.Xjview8.0software gets the active area image.P < 0.05 for the difference was statistically significant.Results:1.The mean volume of activation volume of orbital frontal cortex in the tinnitus group was 140.63 ± 122.703,and the mean volume of the activation volume of the orbital frontal cortex in the control group was 87.00 ± 112.936.There was no significant difference between the two groups(Z=-1.826,P > 0.05).The mean volume of amygdala in the tinnitus group was 7.17 ± 14.708,and the mean volume of activation volume of amygdala in the control group was 9.27 ± 15.797,.There was no significant difference between the two groups(Z =-0.688,P > 0.05).The mean volume of activated volume of the anterior cingulate gyrus in the tinnitus group was57.87 ± 85.864,and the mean volume of activation volume of the anterior cingulate gyrus in the control group was 36.50 ± 47.073.There was no significant difference between the two groups(Z =-0.133,P > 0.05).2.There was no correlation between the activation volume of orbital frontal cortex in the tinnitus group and the score of THI,SAS and SDS(rs = 0.201,P =0.346;rs =-0.390,P = 0.059;rs =-0.242,P = 0.255).There was no correlation between the activation volume of amygdala in the tinnitus group and the score of THI,SAS and SDS(rs = 0.372,P = 0.073;rs = 0.267,P = 0.207;rs = 0.156,P = 0.466),There was no correlation between the activation volume of anterior cingulate gyrus in the tinnitus group and the score of THI,SAS and SDS(rs =-0.001,P = 0.998;rs =-0.345,P = 0.099;rs =-0.230,P = 0.279).Conclusion:1.FMRI can be used as an objective test method for the severity of tinnitus.2.Idiopathic tinnitus patients with orbital frontal cortex,anterior cingulate gyrus activation volume than normal human orbital frontal cortex,anterior cingulate gyrus activation volume larger.Idiopathic tinnitus patients with amygdala activation volume than the normal activation of small size.3.Patients with idiopathic tinnitus orbital frontal cortex,anterior cingulate gyrus and amygdala activation volume and tinnitus severity,anxiety and depression has nothing to do.
