Objective: To evaluate the clinical efficacy and feasibility of platelet-rich fibrin(PRF)in promoting soft tissue healing and bone tissue repair,and to observe the integration and clinical therapeutic effect,PRF or oral repair membrane combined with bone powder were used to preserve the site in the jaw bone defects area around implants in immediate implantation,implant implantation during bone splitting and maxillary sinus ascension.Methods: 38 cases with dental implants in the molar region were selected as study objects.There were 12 cases of immediate implantation,16 cases of implant implantation during bone splitting and 10 cases in maxillary sinus ascension.Each operation was randomly divided into PRF group and biofilm group.19 cases in each group were filled with PRF membrane combined with iliac bone powder and oral repair membrane combined with bone powder.Observing these three sites of preservation of postoperative pain situation,soft tissue healing,and combined with the prognosis of implants,measurement of alveolar bone changes by using the Planmeca Dimaxis Pro 4.5.0(ZH)software analysis system to evaluate the effect of the two materials.Results:(1)At 24 hours after operation,The PRF group and the biofilm group showed pain in all three operations.Gingival index(GI)showed that there was no inflammatory reaction in 9 cases in two group,but the incidence of adverse reactions was same(P > 0.05).After 10 days and1,3 months,all the soft tissues around the implants were well formed,no postoperative complications were found,and the implants healed without any exposure to theimplants at the initial stage.(2)PRF group and biofilm group were implanted immediately 3months after operation.The alveolar bone density was maintained at the same time of implant implantation(P > 0.05).The effect of PRF group on promoting the formation of new bone was better than biofilm,and both of them could maintain the alveolar bone height.(3)In the immediate implantation,PRF group could maintain bone density and bone height.Both bone splitting and maxillary sinus ascension could maintain the density of maxillary bone and inevitably prevent alveolar bone resorption.(4)Among the three surgical methods,the PRF group was more suitable for the preservation of the site of jaw defect after immediate implantation,and the effect of biofilm group was the same as that of the biofilm group.Conclusion:(1)PRF and oral repair membrane can be used to repair the mandibular defect around implants.The adverse reaction rate is low,the healing effect of soft tissue is good,and the gingival tissue around the barrier defect area can be used to guide the formation of new bone.Reconstruct the bone tissue of the jaw bone defect.(2)In maxillary sinus floor elevation,the effect of PRF in promoting the formation of new bone is superior to that of the oral repair film,and the effect of PRF in immediate implant is better for immediate implant,simultaneous implant implantation and maxillary sinus floor elevation.(3)PRF has the merit of simplicity,safety and low price,and it is reliable in the repair of jaw defects.However,the long-term effect of PRF on bone regeneration needs to be studied.
