Analysis Of Clinical Features And Trends Of Infective Endocarditis

Objective: Infectious endocarditis is an infectious disease with high mortality.In recent years,the guidelines have been updated gradually on the prevention,diagnosis and treatment of infectious endocarditis,according to the epidemiological changes of IE through the retrospective study.The purpose of this study is to review and compare the clinical characteristics of IE on clinical in the first hospital of China Medical University,so as to obtain the changes of pathogenic bacteria of IE in this region and provide a reference to improve the clinical treatment effect.Methods: In this study,we obtained the clinical date of 634 patients diagnosed with infective endocarditis from 2004 to 2018 by searching the Hospital Information system of our hospital,and then definite the diagnose by the modified Duke criteria.Operation rate,hospitalization cost,clinical outcomes and other parameters were analyzed.Chi-square test was used for counting data,and T test was used for measurement data.Results: There were 418 males and 216 were females in the cohort,with a male to female ratio of 1.9:1.The mean age was(47.7±16.2)years.The most common age is 50 to 60 years.173 patients(27.3%)had congenital heart diseases and 74(11.7%)patients had history of heart surgery.The proportion of congenital heart disease decreased year by year.513(80.9%)patients were infected a single position.121(19.1%)cases were infected with multiple positions.The positive rate of blood culture was on the rise.The most common pathogen is streptococcus,which main type was streptococcus aeruginosa,which proportion was decreased obviously.The kind of pathogens increased and distributed diversely.Heart failure,renal failure and pulmonary embolism increased.The rate of surgery is increasing.On clinical,the amount of antibiotics,advanced antibiotics and the duration of antibiotics are increasing.The cure rate and the death rate also increased.Heart surgery decreased death rate significantly.Conclusion: The patients of IE are increasing year by year and multiple valve infections are more common.Because of the diversification of pathogens and higher risk of complications,the treatment on clinical become more difficult.Thus,early diagnosis and effective therapy with regular antibiotic is still the foundation of IE.Timely cardiac surgery can also improve the cure rate of IE.
